布鲁杆菌病患者的临床诊断及随访调查研究

摘　　要：目的通过比较细菌培养、试管凝集试验(SAT)和虎红平板凝集实验(RBPT)对就诊患者的检测结果,探讨不同实验室方法对布鲁杆菌病的临床诊断效率,并通过诊后随访探讨疾病的演进过程。方法选择2012年1月~2014年12月收治的布鲁杆菌病患者112例作为研究对象,对所有患者均进行细菌培养、试管凝集试验(SAT)和虎红平板凝集实验(RBPT)检测,并于治疗后3、6、12个月进行跟踪随访,观察患者临床症状及体征变化情况。结果 112例标本细菌培养均为阳性,SAT法检测110例为阳性,2例阴性;RBPT法检测109例阳性,3例阴性,3种检测方法对布鲁杆菌的检出率差异无统计学意义(P>0.05)。112例患者中1例无明显感染接触史,111例患者有直接接触或间接接触牲畜及其相关产品史;临床表现多为畏寒、发热、发力、多汗等。结论布鲁氏杆菌感染患者临床表现具有多样性特点,3种方法相互结合,可以提高布鲁杆菌检出率,提高临床诊断效率和准确率,同时通过定期随访有利于掌握患者疾病变化情况,可及时发现复发患者,值得临床重视。Objective By comparing the bacterial culture,tube agglutination test(SAT)and tiger red plate agglutination test(RBPT)the detection results of hospitalized patients and to explore different laboratory methods for bloom bacillus disease clinical diagnosis efficiency,and through the clinical follow-up after investigate the evolution process of the disease.Methods January 2012~December 2014 by brookings coli patients treated 112 cases as the research object,for all patients with bacterial culture,tube agglutination test(SAT)and tiger red plate agglutination test(RBPT),and 3,6,12 months after treatment for follow-up,the changes of clinical symptoms and signs were observed.Results 112 cases of bacterial culture specimens are all positive,110 cases were positive for the SAT method and 2cases of negative;RBPT method to detect 109 cases of positive and negative in 3cases,three detection methods for bloom bacteria detection rate no difference was not statistical significant(P>0.05).112 cases of patients with no obvious contact infection,1case in 111 patients with history of direct contact or indirect contact with animals and their related products;Clinical expression of chills,fever,more power,sweat,etc.Conclusion Clinical manifestations of brucella bacillus infection patients have the characteristics of diversity,the three methods in combination with each other,can improve the brookings bacillus detection rate,improve the clinical diagnosis efficiency and accuracy,at the same time through regular follow-up is helpful to grasp the changes in patients with disease,can timely find relapse patients,worthy of clinical attention.
