瘢痕子宫再次妊娠分娩方式临床分析

摘　　要：目的:分析瘢痕子宫再次妊娠不同分娩方式的临床效果,为产妇再次妊娠分娩选择最佳方式提供依据。方法:选取本院2013年6月-2015年6月收治的瘢痕子宫再次妊娠分娩产妇100例为研究对象,对其临床资料进行回顾性分析。比较瘢痕子宫再次妊娠剖宫产患者及阴道试产成功患者的分娩结局,同时比较上述阴道试产成功的产妇与同期非瘢痕子宫阴道分娩产妇的妊娠结局。结果:100例瘢痕子宫再次妊娠分娩产妇中,阴道试产成功率为48.0%,再次剖宫产率为52.0%,再次剖宫产患者的产时出血量、住院时间均明显多于阴道分娩组,差异均有统计学意义(P<0.05)。且瘢痕子宫阴道试产成功的患者与非瘢痕子宫阴道分娩产妇的妊娠结局比较,差异无统计学意义(P>0.05)。结论:在严格掌握阴道分娩指征且严密监测产程的情况下,瘢痕子宫再次妊娠行阴道分娩是安全可行的,在此条件下可以提高阴道分娩率,从而进一步降低再次剖宫产发生率。Objective：To analyze the clinical effect of different delivery mode in the pregnant women with scar uterus,and to provide the basis of the best delivery mode selection for the pregnant women with scar uterus.Method：One hundred cases of pregnant women with scar uterus who were treated in our hospital from June 2013 to June 2015 were retrospectively analyzed.The pregnancy outcomes between the vaginal delivery patients with scar uterus and the re-caesarean patients with scar uterus,as well as the vaginal delivery patients with scar uterus and the homochronous vaginal delivery healthy puerperae were compared. Result：In these 100 cases of uterine scar pregnancy,the success rate of vaginal delivery was 48.0%,the re-cesarean section rate was 52.0%,the bleeding, hospitalization time of re-cesarean section patients were higher than which of the vaginal delivery patients,the differences were statistically significant（P〈0.05）. The difference had no statistic significant on the pregnancy outcomes between the vaginal delivery patients with scar uterus and the homochronous vaginal delivery healthy puerperae（P〉0.05）.Conclusion：Under a strict control of vaginal delivery indications and a strict monitoring of delivery process,the vaginal delivery is safe and feasible for pregnant women with scar uterus,which can improve the vaginal delivery rate,so as to further reduce the incidence rate of cesarean section.
