达泊西汀联合行为疗法治疗原发性早泄的临床研究

摘　　要：目的:观察达泊西汀联合行为疗法治疗原发性早泄的临床疗效。方法:将门诊82例诊断为原发性早泄患者分成两组(A组、B组),其中A组(40例)单纯给予达泊西汀治疗,B组(42例)给予达泊西汀联合行为疗法治疗。两组疗程均为24周。分别于治疗前、治疗4周、治疗12周、治疗24周时对患者阴道内射精潜伏期(IELT)、早泄诊断标准评分(PEDT)及不良事件进行评估、比较。结果:与治疗前相比,2组治疗4周、12周、24周时平均IELT比治疗前明显增加(P<0.01),但B组提高程度明显优于A组(92.0s、137.9s、232.7s、370.7s vs.85.0s、84.8s、130.7s、160.0s,P<0.01);与治疗前相比,2组治疗4周、12周、24周时平均PEDT评分比治疗前明显降低(P<0.01),但B组降低程度明显优于A组(19.56、16.0、11.96、7.92 vs.20.4、18.16、15.88、14.68,P<0.01)。治疗结束后,A组中无患者达到正常射精的评分标准(PEDT score≤8);而B组中81%的患者达到正常的射精标准(PEDT score≤8)。不良事件总发生率无明显统计学差异(A组:15%;B组:16.7%,P>0.05)。结论:达泊西汀联合行为疗法治疗原发性早泄疗效明显优于单纯服用达泊西汀治疗,能恢复大部分患者正常的射精功能,值得临床推广。Objectives： To observe the clinical curative effect of dapoxetine combined with behavioral treatment in the treatment of primary premature ejaculation. Methods： 82 patients of primary premature ejaculation were randomly divided into two groups. Group A（ 40 cases） was treated with dapoxetine only and Group B（ 42 cases）was treated with dapoxetine combined with behavioral treatment,all for 24 weeks. The mean PEDT scores and mean IELT were evaluated before and at 4 week,12 week,and 24 week,respectively. Results： From baseline to 4-,12- and 24- week evaluation,both groups experienced a significant（ P〈0. 01） increase in mean IELT and decrease in mean PEDT score,but patients in group A showed a significantly lower increase in mean IELT（ 85. 0sec;84. 8sec; 130. 7sec; 160. 0sec vs. 92. 0sec; 137. 9sec; 232. 7sec; 370. 7sec,respectively; P〈0. 01） and a significantly lower decrease in mean PEDT score（ 20. 4; 18. 16; 15. 88; 14. 68 vs. 19. 56; 16. 0; 11. 96; 7. 92,respectively; P〈0. 01） than those in group B. At 24- week evaluation,no patient in group A reached a PEDT score≤8（ absence of PE） as opposed to 81% of patients in group B. There was no difference between groups in TEAEs rate（ 15% vs. 16. 7%; P〉0. 05）. Conclusion： Dapoxetine combined with behavioral treatment helps improve the curative effect of the treatment of primary premature ejaculation,and therefore deserves wide clinical application.
