直肠高分辨磁共振与经直肠腔内超声对T2、T3期中低位直肠癌的诊断效能比较

摘　　要：目的探讨直肠高分辨率磁共振(HD MRI)及经直肠腔内超声(trans-rectal ultrasound,TRUS)在T2、T3期中低位直肠癌术前T分期中的应用价值。方法收集广西医科大学第一附属医院结直肠肛门外科2013年12月至2014年12月共53例经病理证实的T2、T3期中低位直肠癌患者临床资料,使用直肠HD MRI及TRUS进行分期诊断,并与术后病理T分期进行对照,比较二者准确率。结果直肠HD MRI对中低位直肠癌术前T2、T3分期准确率分别为61.54%和95%,TRUS则为69.23%和82.50%。直肠HD MRI及TRUS对T2及T3期中低位直肠癌病理分期总体准确率为86.79%和79.25%。结论直肠HD MRI及TRUS对T2、T3期中低位直肠癌术前T分期均有较高的诊断准确性,在判断患者是否需行新辅助放疗时有较高的参考价值。Objective To compare the ability of rectal high-resolution MRI（HD MRI） and trans-rectal ultrasound（TRUS） in staging T2 and T3 mid-low rectal cancer with pathology.Methods From December 2013 to December 2014,fifty-three cases who were pathologically confirmed staging T2 and T3 mid-low rectal cancer and received preoperatively scan by 3.0T HD MRI and TRUS in the department of colorectal and anal surgery of 1st affiliated hospital of Guangxi Medical University were included in this retrospective study. Image signs were extracted from HD MRI and TRUS, then the diagnostic accuracy of HD MRI and TRUS were assessed and compared with histopathological classification. Result The T-stage diagnostic accuracy of T2 and T3 by using HD MRI was 61.54% and 95% respectively, while TRUS was 69.23% and 82.50%. The overall diagnostic accuracy between HD MRI and TRUS was 86.79% and 79.25%. Conclusion The HD MRI and TRUS for the staging T2 and T3 mid-low rectal cancer has high diagnostic accuracy and contributes to determine whether it was suitable for patients undergoing neo-adjuvant radiation.
