摘 要:目的对从广州白云国际机场口岸入境的1例自委内瑞拉归国发热人员进行寨卡病毒实验室检测。方法同时使用自主研制的和中国疾病预防控制中心发放的实时荧光RT-PCR检测试剂,对发热人员的血清、全血、尿液、唾液和咽拭子等多种样本进行寨卡病毒核酸检测;对唾液样本进行寨卡病毒RT-PCR扩增和序列分析。结果入境第一天上午采集的血清、全血和咽拭子样本的实时荧光RT-PCR结果显示寨卡病毒核酸阳性,当天下午采集的血清样本检测结果显示核酸弱阳性,第二天采集的血清样本为寨卡病毒核酸阴性,但尿液和唾液样本仍为阳性,且唾液中病毒载量较高。RT-PCR结果显示,唾液样本扩增出了预期片段,测序结果表明该片段与巴西寨卡病毒毒株相应序列的同源性为99.8%。中国疾病预防控制中心复核检测确认为寨卡病毒核酸阳性。结论根据流行病学史、临床表现和病例样本核酸检测情况,确诊该入境人员为口岸截获首例、广东省首例、我国第二例输入性寨卡病毒感染病例。Objective To detect Zika virus(ZIKV) in a suspected case returning from Venezuela and entry from Guangzhou Baiyun International Airport. Methods A variety of samples including serum,whole blood,urine,saliva and nasal swabs were collected. ZIKV RNA was detected by using real-time fluorescence RT-PCR methods provided by both in house and Chinese Centre for Disease Control and Prevention(China CDC). The saliva sample was detected by RT-PCR,and sequence analysis was performed. Results Real-time fluorescent RT-PCR results showed that serum,whole blood and nasal swabs collected in the morning of the first day of entry were positive,and serum sample collected in the afternoon was weakly positive. Serum sample collected in the second day was negative while urine and saliva sample were positive. A specific fragment had been amplified with saliva sample by RT-PCR,and sequence analysis showed 99.8% homologous to the corresponding sequence of Brazil ZIKV(Gen Bank No. Be H819966).China CDC reviewed the specimens ZIKV RNA positive. Conclusion According to epidemiology,clinical manifestations and RNA detection of the samples,the suspected case was confirmed to infect Zika virus,being the first case at port,the first case in Guangdong province and the second case in China.
