颅内压监测指导严重脑外伤临床治疗探讨

摘　　要：目的：探讨动态颅内压（ ICP）监测在严重脑外伤治疗方面的应用价值。方法回顾性分析济源市人民医院2012年8月至2015年1月收治的严重脑外伤患者的临床资料，其中37例患者选择使用ICP监测者作为观察组，另选择同期入院的37例严重脑外伤但未选用ICP监测的患者作为对照组。对比观察两组患者治疗后CGS评分、治愈时间、感染率及病死率等情况。结果观察组患者治疗后GCS评分明显高于对照组患者，且治愈时间短于对照组，两组治疗后GCS评分与治愈时间比较差异有统计学意义（ P﹤0.05），两组患者病死率与感染率比较差异未见统计学意义（ P﹥0.05）。结论严重脑外伤患者采用ICP监测，可以迅速、客观、准确地了解患者ICP变化情况，有利于医护人员对患者病情进行判定，并给予对症处理，对提高临床疗效，改善预后具有重要的参考价值。objective To investigate the application value of dynamic intracranial pressure （ ICP）monitoring in the treatment of severe brain trauma. Methods The clinical data of patients With severe brain trauma from August 2012 to January 2015 Were retrospectively analyzed. There Were 37 pa-tients（ observation group）Were given ICP monitoring in this study,and 37 patients With severe brain trauma and didn’t choose to use ICP monitoring Were selected as control group during the same period. Patients’CgS score,healing time,infection rate,fatality of the tWo groups after treatment Were contras-ted and observed. Results gCS score of patients in the observation group Was significantly higher than that in the control group after treatment,and the healing time Was significantly shorter than that in the control group. The differences of gCS scores and the healing time betWeen the tWo groups after the treat-ment had significant differences（ P﹤ 0. 05 ）. There Were no significant differences in the fatality rates and infection rates betWeen the tWo groups（P ﹥ 0. 05）. Conclusions It can acquaint patients’chan-ges of ICP quicKly,objectively and accurately for patients With severe brain trauma to use ICP monito-ring. It can help the medical care personnel to determine patients’conditions,and give symptomatic treatment. It has important reference value of enhancing the clinical effect and improving the prognosis.
