摘  要:目的:探究胎儿颈项透明层厚度超声(NT)检测联合孕中期四维彩超诊断胎儿畸形的应用价值。方法:入选我院2013-01~2015-12产检孕妇1230例,予以产前胎儿颈项透明层厚度超声(NT)检测联合孕中期四维彩超监测与检查,对检查结果进行分析并随访至产后。结果:孕11~14wk 1230例妊娠产检者中,确诊异常胎儿47例(3.8%),NT检测出异常胎儿45例(3.7%),诊断准确率95.7%,包括淋巴管囊瘤12例(胎儿颈项透明层厚度增厚),胎死宫内20例,胎儿异常发育13例;继续妊娠的1230例孕妇孕22~28wk四维彩超检测中,确诊异常胎儿50例(4.1%),四维彩超检测出异常胎儿50例(4.1%),诊断准确率100.0%。结论:孕11~14wk进行胎儿颈项透明层厚度超声检测有助于评估胎儿唐氏综合征的风险,孕22~28wk进行四维彩超监测有助于发现胎儿结构异常,二者联合检测,对筛查胎儿先天畸形具有重要的临床意义。Objective: To evaluate the application value of fetal nuchal translucency( NT) obtained in first trimester combined with four- dimensional( 4D) ultrasound applied in second trimester in detecting fetal deformities. Methods: A total of 1230 fetuses were selected which accepted NT inspection and 4D ultrasound systematic examing between January 2013 and December 2015 in our department.The detecting results were analyzed and followed up to the fetus after birth. Results: During 11 ~ 14 weeks gestation,47( 3. 8%) out of 1230 fetues were diagnosed confirmedly with deformities,with 45( 3. 7%) fetus( 12 cases had neck lymphatic hygroma,20 cases with intrauterine fetal death,13 cases with abnormal development) were discovered by NT inspection,so the diagnostic accuracy was 95. 7%;During 22 ~ 28 weeks gestation,the 1230 cases were given 4D ultrasound systematic examination,50( 4. 1%) fetuses were diagnosed confirmedly with deformities,all of which were detected by 4D ultrasound,so the diagnostic accuracy was 100. 0% 。Conclusion: Fetal NT inspection helps to assess the risk of fetal down syndrome in 11 ~ 14 weeks gestation,and the 4D ultrasound systematic examination can help find fetal structural abnormalities. The combination of fetal NT inspection and 4D ultrasound has important clinical significance for screening fetal congenital anomalies.
