摘  要:目的:描述腹腔镜下精索静脉曲张患者精索内静脉、睾丸动脉与淋巴管的数量及相互关系。方法:选取就诊的精索静脉曲张患者57例(年龄18~45岁)行腹腔镜下精索内静脉高位结扎术,术中记录精索内静脉、睾丸动脉与淋巴管的数量,记录三者之间的位置关系。结果:精索内静脉的数量为(3.3±1.2)条,睾丸动脉的数量为(1.4±0.9)条,精索淋巴管的数量为(4.3±1.1)条,两侧精索内静脉、睾丸动脉、精索淋巴管数量没有统计学差异(P〉0.05)。睾丸动脉常位于精索内静脉的外侧,并与其缠绕,而精索淋巴管则多居于静脉之间。结论:精索内的精索内静脉、睾丸动脉与淋巴管的解剖关系具有一定特征性,腹腔镜下可以区分精索内静脉、睾丸动脉与淋巴管,为手术治疗精索静脉曲张建立解剖学基础。Objective: To study the numbers and locations of spermatic veins,testicular arteries,and lymphatic vessels in the spermatic cord of the varicocele patient under the laparoscope. Methods: Fifty-seven varicocele patients received laparoscopic ligation of spermatic veins,during which we recorded the numbers and observed the locations of spermatic veins,testicular arteries,and spermatic lymphatic vessels. Results: During the surgery,we identified 3. 3 ± 1. 2 spermatic veins,1. 4 ± 0. 9 testicular arteries,and4. 3 ± 1. 1 spermatic lymphatic vessels. No statistically significant differences were observed between the two side in the numbers of the spermatic veins,testicular arteries and spermatic lymphatic vessels( P〉0. 05). The testicular arteries were seen on the exterior of the spermatic veins and winding around them,while the spermatic lymphatic vessels mostly between the veins. Conclusion: The spermatic veins,testicular arteries,and lymphatic vessels in the spermatic cord of the varicocele patient have their specific anatomic characteristics. Laparoscopic identification of these vessels may contribute to the surgical treatment of varicocele.
