摘  要:目的:探析应用瞬时波强技术评价原发性高血压患者颈动脉弹性功能的作用。方法:选取收治的90例老年原发性高血压患者作为研究组,同期于我院接受健康体检者90例作为对照组,两组行血压测量、超声心动图检查及WI测量。结果:高血压组W1为(13.1±6.5)×10~3mm Hg·m/s^3,高于对照组(8.5±2.4)×10~3mmHg·m/s^3;W2为(3.9±0.6)×10~3mm Hg·m/s^3,高于对照组(2.3±0.5)×10~3mm Hg·m/s3,组间比较,差异均有统计学意义(P<0.05)。高血压组W1与年龄呈正相关,对照组W1与各因素无显著相关性;高血压组W2与脉压、颈动脉内中膜厚度(IMT)呈正相关,对照组W2与颈总动脉收缩期峰速度(PSV)呈负相关。结论:原发性高血压患者W1、W2明显高于健康人群,可评价患者颈动脉弹性及血流动力学变化情况进而预测患者疾病进展及转归、预防脑血管意外。Objective To explore the application of instantaneous wave intensity technique in the evaluation of carotid artery elastic function of patients with essential hypertension. Method 90 cases of elderly patients with essential hypertension were selected as the research group and 90 cases of healthy persons came to the hospital during the same period as the control group. Both of the two groups received blood pressure measurement,echocardiography examination and WI measurement. Results The W1 of the hypertension group was( 13. 1 ± 6. 5) × 103 mm Hg·m / s3,which was higher than( 8. 5 ± 2. 4) × 10~3 mm Hg·m / s^3 of the control group and W2( 3. 9 ± 0. 6) × 103 mm Hg·m / s3 was also higher than( 2. 3 ± 0. 5) × 103 mm Hg·m / s^3 of the control group. The comparison between the groups were statistically significant( P〈0. 05). W1 of the hypertension group was positively correlated with age while there was no significant correlation between W1 of the control group and other factors. W2 of the hypertension group was positively related to pulse pressure and IMT while the W2 of the control group was negatively correlated with PSV. Conclusion W1 and W2 of the patients with essential hypertension are significantly higher than those of healthy people. The disease progression and prognosis can be predicted by evaluating the changes of carotid artery elasticity and hemodynamics in patients,preventing cerebrovascular accident.
