摘 要:目的观察补气益精生血中药对中间型β地中海贫血患儿p38MAPK信号通路的影响。方法采用随机对照双盲临床研究,入选β地贫患儿50例随机分配进入中药治疗组及安慰剂对照组,中药治疗组患儿口服黄芪、党参及龟板颗粒,对照组口服安慰剂颗粒,疗程12周。观察血常规及胎儿血红蛋白比例等血液学指标。并以实时荧光定量PCR反应检测p38MAPK基因m RNA水平,以Western Blot方法检测p38MAPK和磷酸化p38MAPK蛋白水平。结果 12周疗程结束后,中药组患儿血红蛋白值为85.90±13.30 g·L^(-1),对照组为75.80±9.50 g·L^(-1),两组比较差异有统计学意义(t=6.310,P=0.004)。中药组治疗后胎儿血红蛋白比例较治疗前亦有显著提升(58.13%±20.52%vs.66.80%±21.63%,t=-4.972,P<0.001)。实时荧光定量PCR及Western Blot实验结果显示中药治疗对p38MAPK基因表达无明显影响,但能显著提升磷酸化p38MAPK蛋白水平(0.33±0.19 vs.0.81±0.40,t=-7.652,P<0.001)。结论补气益精生血治法方药治疗小儿β地贫具有较好疗效,其分子机制可能与激活p38MAPK信号通路从而诱导胎儿血红蛋白生成有关。Objective To explore the influence of Chinese medicine(CM) for replenishing Qi and producing essence and blood on p38 MAPK signaling pathway in children with β- thalassemia. Methods A randomized controlled double- blind clinical study was carried out in 50 pediatric patients with β- thalassemia. The subjects were randomly assigned to Chinese medicine(CM) group and placebo control group. The children in CM group were orally administrated Radix Astragali, Radix Codonopsis and Tortoise plastron granules, and the placebo group was orally were administrated placebo granules for 12 weeks. The parameters of blood routine test and fetal hemoglobin(Hb F)proportions were assessed before and after treatment. In addition, the expression level of p38MAPK m RNA was detected by using real- time fluorescent quantitative RT- PCR, and the p38MAPK and phosphorylated p38MAPK protein levels were analyzed by using Western blot method. Results After treatment for 12 weeks, the mean Hb F value in children of CM group was 85.90 ±13.30 g·L^(-1),higher than 75.80±9.50 g·L^(-1) in placebo control group(t=6.310,P=0.004). Compared with that before treatment,mean Hb F proportion of CM group rose significantly(58.13 %±20.52 % vs 66.80 %±21.63 %,t=- 4.972,P〈0.001). The results of real- time RT- PCR indicated that Chinese medicine had no marked effect on p38MAPK gene expression, but could significantly increase phosphorylated p38MAPK protein levels(0.33 ±0.19 vs 0.81 ±0.40, t=- 7.652, P〈0.001). Conclusion Chinese medicine for replenishing Qi and producing essence and blood is effective for the treatment of pediatric patients with β- thalassemia.The molecular mechanisms may involve in inducting fetal hemoglobin formation through activating p38MAPK signaling pathway.
