假性动脉瘤彩色多普勒超声影像诊治分析

摘　　要：目的:探讨彩色多普勒超声在假性动脉瘤(PSA)诊治中的价值。方法:对32例假性动脉瘤患者进行彩色多普勒超声检查,观察瘤腔内部回声、破口、血流信号及频谱特征;对其中27例PSA行超声引导加压或注射凝血酶栓塞疗法。结果:32例PSA经造影、治疗及病理证实。瘤腔内见起自载瘤动脉呈喷射状五彩镶嵌动脉血流信号,在瘤腔内呈红蓝漩涡状改变,破口处可测得"双期双向"动脉血流频谱。27例经超声引导压迫、穿刺抽血及注射凝血酶栓塞治疗效果满意,无并发症。结论:彩色多普勒超声在假性动脉瘤诊治中准确度和治愈率高,在临床诊疗中具有重要应用价值。Objective： To evaluate the clinical value of color Doppler flow imaging（ CDFI） in diagnosis of pseudoaneurysm（ PSA）. Method： 32 cases of PSA were examined by CDFI,and their sonographic characteristics were observed.27 cases of these received ultrasound guided compression or thrombin injection therapy. Results： The sonographic diagnoses were coincident with the findings of surgeries or angiographies or pathology. CDFI showed turbulent flow in the pseudoaneurysms and demonstrated the to and fro pectrum during systole and diastole at neck of the pseudoaneurysm. 27 patients with PA following catheterization were cured successfully with ultrasound-guided compression,puncture hemospasia or thrombin embolization therapy. Conclusion： Color doppler ultrasound in the diagnosis and treatment of PSA in accuracy and high cure rate,and has important application value in the clinical diagnosis and treatment.
