超敏C反应蛋白在阻塞性睡眠呼吸暂停低通气综合征中的检测意义

摘　　要：目的目的探析阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血浆超敏C反应蛋白(hs-CRP)的水平变化以及hs-CRP与肥胖、睡眠呼吸参数的关系。方法分析2013年11月至2015年3月在我院确诊并未接受治疗的57例OSAHS患者(OSAHS组)和20例多导睡眠图(PSG)监测正常者(对照组)的临床资料。参照呼吸暂停低通气指数(AHI)将OSAHS患者分为轻度组(18例)和中重度组(39例)两组。参照世界卫生组织(WHO)公布的相关规定将77例受试者分成肥胖组(18例)和非肥胖组(59例)两组。比较OSAHS组和对照组的一般资料、睡眠呼吸参数、hs-CRP水平、肥胖与非肥胖者以及OSAHS组内轻、中重度组的hs-CRP水平。结果 OSAHS组和对照组一般资料间差异不明显(P>0.05)。OSAHS组患者的AHI明显比对照组高(P<0.01),而最低血氧饱和度(LSpO2)明显低于对照组(P<0.01)。OSAHS组血浆hs-CRP水平明显高于对照组(P<0.05);且OSAHS组内轻度组血浆hs-CRP水平明显低于中重度组(P<0.05)。肥胖组患者血浆hs-CRP水平明显高于非肥胖组(P<0.01)。结论 OSAHS患者的血浆hs-CRP水平明显上升,其与肥胖、睡眠呼吸参数关系密切。Objective Our retrospective study was aimed toanalyze the changes of the levels of high sensitivity C-reactive protein（hs-CRP）in patients with Obstructive sleep apnea-hypopnea syndrome（OSAHS）and the relationship between hs-CRP and obesity,sleep breathing parameters.Methods Clinical data of 57 patients with OSAHSreceived treatment at our hospitalfrom 2013 to 2015were analyzed as OSAHS group.20 healthy cases were enrolled as control group.OSAHSgroupwere divided into two subgroups according to apnea hyponea index（AHI）,mildsubgroup（18cases）and moderate and severesubgroup（39cases）.In addition,OSAHSgroupwere also divided into other two subgroups according to the BMI values（WHOstandard）,obesity group（18cases）and non-obesity group（59cases）.The following index were compared between the groups：general information,sleep breathing parameters,hs-CRP and so on.Results There was no statistical difference in general information between OSAHS group and control group（P〉0.05）.The AHI values in OSAHS group were obviously higher than those in control group（P〈0.01）,and the levels of LSpO2 in OSAHS group were obviously lower than those of control group（P〈0.01）.The level of hs-CRP in OSAHS group were obviously higher than those in control group（P〈0.05）,and the level of hs-CRP in moderate and severe group were obviously higher than theseof mild group（P〈0.05）.The level of hs-CRP in obesity group was obviously higher than that non-obesity group（P〈0.01）.Conclusion The level of hs-CRP in OSAHS group increased significantly.The hs-CRP level has close correlation with obesity and sleep breathing parameters.
