不稳定性心绞痛患者超敏肌钙蛋白T与冠状动脉SYNTAX评分相关性分析

摘　　要：目的探讨不稳定性心绞痛患者超敏肌钙蛋白T（hs-cTnT）水平与冠状动脉病变SYNTAX评分之间的关系。方法将157例不稳定性心绞痛患者根据冠状动脉造影结果 SYNTAX评分分为低分组（评分〈22分,97例）和中高分组（评分≥22分,60例）,对比分析两组患者的临床资料和血清hs-cTnT表达水平,应用双变量相关分析、多元线性回归和Logistic回归分析hs-cTnT与冠状动脉SYNTAX评分之间的相关性。结果 （1）中高分组血清中hs-cTnT水平明显高于低分组,差异有统计学意义（P〈0.01）;（2）患者血清中hs-cTnT水平与冠状动脉SYNTAX评分有相关性（r=0.661,P〈0.01）;（3）多元线性回归分析显示hs-cTnT水平与冠状动脉SYNTAX评分独立相关（β=0.077,P〈0.01）;（4）Logistic回归分析显示hs-cTnT是冠状动脉SYNTAX评分的独立预测因子（OR=1.148,95%CI：1.087~1.212;P=0.001）。结论在不稳定性心绞痛患者中,hs-cTnT水平升高可能提示患者冠状动脉病变严重程度。Objective To investigate the correlation between serum high-sensitivity cardiac troponin T（hs-cTnT）and SYNTAX score in the patients with unstable angina pectoris.Methods According to the score of SYNTAX,157 patients with unstable angina pectoris were divide into the intermediate-high score group（≥22points,60cases）and low score group（22points,97cases）.Then the clinical data and the serum hs-cTnT expression level were compared between the two groups.In addition,the bivariate correlation,multivariate stepwise regression and Logistic regression analysis were used to analyze the relationship between hs-cTnT and SYNTAX score.Results（1）The level of hs-cTnT in the intermediate-high score group was significantly higher than that in the low score group（P0.01）;（2）the SYNTAX score was correlated with the hs-cTnT level（r=0.661,P0.01）;（3）the multiple linear regression analysis showed that hs-cTnT correlated with SYNTAX score independently（β=0.077,P0.01）;（4）the Logistic regression showed that hs-cTnT was an independent predictors of SYNTAX score（OR=1.148,95%CI：1.087-1.212,P=0.001）.Conclusion In unstable angina pectoris patients,the elevated hs-cTnT level may indicate the severity of coronary artery disease.
