某院静脉用药调配中心退药情况分析及建议

摘　　要：目的分析上海交通大学附属第六人民医院东院静脉用药调配中心退药现状及原因,以提高工作效率,促进临床合理用药。方法对医院静脉用药调配中心2014-01～2014-12的电子退药医嘱进行回顾性分析。结果 2014-01～2014-12医院静脉用药调配中心共收到输液医嘱282 009帖,其中退药9 153帖,占输液总量的3.25%。妇产科病区、ICU病区、儿科病区的退药率均较高,分别为8.23%,7.71%,7.15%。诊疗方案改变（49.08%）和出院、转科、死亡（45.93%）是导致退药的主要原因。调节水、电解质及酸碱平衡类药退药比例最高,为32.54%;其次为抗菌药物（26.37%）和中药注射剂（14.31%）。结论建议采取完善静脉用药调配中心退药管理制度,提高药师和医生的用药知识水平,加强医生、患者、药师间的沟通和医院的信息系统管理等措施,有效减少退药发生。Objective To analyze the current situation and causes of the drug repercussion in pharmacy intravenous admixture service（ PIVAS） of Shanghai Jiaotong University Affiliated Sixth People＇s Hospital for improving the work efficiency and promoting the rational clinical drug administration. Methods The electronic prescriptions of drug repercussion in pharmacy intravenous admixture service from January to December 2014 were reviewed in this paper. Results A total of 282 009 electronic prescriptions in PIVAS were collected,and the drug repercussion accounted for 3. 25%（ 9 153 cases）. And the rate of drug repercussion was higher in department of gynaecology and obstetrics,ICU ward,and paediatrics ward with the record of 8. 23%,7. 71% and 7. 15%,respectively. In addition,the major factor of drug repercussion was the change of diagnosis and treatment（ 49. 08%） and the hospital discharge,transferring department and death（ 45. 93%）. The main drugs of drug repercussion were for adjusting water,electrolyte and acid-base balance（ 32. 54%）,followed by antibiotics（ 26. 37%） and herbal injection（ 14. 31%）. Conclusion The manager system of PIVAS need be further improved and the relevant knowledge of pharmacists and doctors should be enhanced. Moreover,the communication among doctor,patient and pharmacist should be also noted with the promotion of management on information system to validly reduce the dug repercussion.
