早期关节内减压加自体红骨髓移植治疗青壮年股骨颈骨折的临床研究及应用

摘　　要：目的:探究早期关节内减压加自体红骨髓移植治疗青壮年股骨颈骨折的临床效果,为治疗青壮年股骨颈骨折提供临床参考和借鉴。方法:选择2012年6月-2014年9月在本院行股骨颈骨折早期闭合或切开复位、空心加压螺钉固定、同时行髋关节内减压加自体红骨髓移植治疗的30例青壮年患者为观察组,2009年6月-2012年5月在本院择期行股骨颈骨折闭合或切开复位、空心加压螺钉固定治疗的30例青壮年患者为对照组。两组患者术前均进行患肢中立位或轻度屈曲内旋位骨牵引,比较两组患者采用不同方法进行手术的治疗效果,分析早期关节内减压加自体红骨髓移植治疗青壮年股骨颈骨折的临床疗效。结果:两组患者经手术治疗后均随访1年,并行动态X线及MRI影像学检查,末次随访结果显示,观察组患者治疗总有效率为93.3%,对照组患者总有效率为80.0%,观察组治疗效果明显优于对照组,比较差异具有统计学意义(U=2.4601,P=0.0139),两组患者经过治疗后,不良反应发生率比较差异有统计学意义(P<0.05)。观察组患者对治疗结果总满意率为86.7%,对照组患者为70.0%,观察组患者满意程度高于对照组,比较差异有统计学意义(U=2.2932,P=0.0218)。结论:早期关节内减压加自体红骨髓移植治疗青壮年股骨颈骨折效果良好,患者术后满意程度高,值得临床推广及应用。Objective：To investigate the clinical results of early intra-articular decompression and autologous red bone marrow transplantation in the treatment of young adults＇ femoral neck fracture,providing clinical reference for the treatment. Method：From June 2012 to September 2014,30 cases of young patients undergoing early closure or open reduction of femoral neck fracture,cannulated compression screws,simultaneously hip decompression and autologous red bone marrow transplantation treatment in our hospital were selected as the observed group,30 cases of young patients undergoing closed or open reduction of femoral neck fracture,cannulated compression screw fixation from June 2009 to May 2012 in our hospital were selected as the control group.All of the patients were given preoperative limb bone supination neutral or slight flexion traction,the therapeutic effect of using different methods in surgery treatment were compared to analysis the clinical efficacy of early intra-articular decompression and autologous bone marrow transplantation in the treatment of young adults＇ femoral neck fractures.Result：Both of the two groups after surgery were followed up for 1 year,along with the dynamic X-ray and MRI imaging examination,at the end of follow-up,the results showed that the therapeutically effective rate of the observation group was 93.3%,while which was 80.0% in the control group,the difference was statistic significant（ U=2.4601,P=0.0139）,after the treatment,the differences of complications rate between the two groups had no statistic significant（P〈0.05）,the satisfaction rate of the observation group were 86.7%,which was higher than 70.0% of the control group,the difference was statistic significant（U=2.2932,P=0.0218）.Conclusion：The early intra-articular decompression and autologous red bone marrow transplantation shows a good effect in the treatment of young adults with femoral neck fracture,the satisfaction rate is significantly increased,it is worthy of promotion and application.
