透明质酸修护生物膜联合吡美莫司乳膏对面部糖皮质激素依赖性皮炎的疗效观察

摘　　要：目的:观察透明质酸修护生物膜联合吡美莫司治疗面部糖皮质激素依赖性皮炎的疗效。方法:将患者按就诊的登记号单双号分组,按试验标准进行筛选排除后,试验组62例,对照组50例纳入统计。试验组患者外用透明质酸修护生物膜联合吡美莫司连续治疗4周,后4周停用吡美莫司,每日外用透明质酸修护生物膜2次。对照组患者仅外用吡美莫司治疗4周,停药后观察4周。分别于治疗前和治疗第2、4、8周进行疗效评估,测定皮肤生理指标并观察复发情况。结果:治疗2周时试验组痊愈率高于对照组(P<0.05),两组显效率差异无统计学意义;治疗第4周试验组痊愈率和显效率均高于对照组(P均<0.05)。治疗第2、4、8周后,试验组经皮水分丢失(TEWL)值均低于对照组,而角质层含水量(WCSC)均高于对照组(P均<0.05)。治疗第8周试验组复发率(6.45%)低于对照组(18%)(P<0.05)。结论:外用透明质酸修护生物膜联合吡美莫司治疗面部糖皮质激素依赖性皮炎疗效优于单纯外用吡美莫司,且降低复发率。Objectives： To evaluate the efficacy of biomembrane containing hyaluronic acid combined with pimecrolimus for the treatment of patients with facial steroid dermatitis. Methods： The patients were divided into two groups according to their medical registration number（odd or even number）. 62 cases met experimental criteria in treatment group, while control group were 50 cases. The treatment group were given the biomembrane containing hyaluronic acid combined with topical pimecrolimus twice daily for 4 weeks, followed by biomembrane alone twice daily for additional 4 weeks. The controls were treated with piinecrolimus alone twice daily for 4 weeks, and followed up for additional 4 weeks. The clinical efficaeies were evaluated before, 2, 4, 8 weeks after treatment. Changes in skin physiology parameters and the recurrences were also analyzed. Results： After 2 weeks of treat- merit, the cure rate in experimental group was higher than that in the control group（P〈0.05）, while the effective rates were no sig- nificant differences between the two groups. After 4 weeks of treatment, both the cure and effective rates were higher in experi- mental group than in the control group（both P〈0.05）. After 2, 4, 8-week treatment, the TEWL rates in the experimental group were lower than that in the control group, while the WCSC values in experimental group were higher than that in the control group（both P〈0.05）. 8 weeks after the treatment, the recurrence rate in the experimental group was lower than that in the control group（6.45% vs. 18%, P〈0.05）. Conclusion： The efficacy of biomembrane containing hyaluronic acid combined with pimecrolimus for the treatment of facial steroid dermatitis is superior to topical pimecrolimus alone, and with lower recurrence rate.
