摘 要:目的探讨窒息患儿脐血血浆D-二聚体(D-D)和血浆降钙素原(PCT)测定的临床价值。方法选取轻度窒息患儿40例(轻度窒息组)、重度窒息患儿30例(重度窒息组),运用Sysmex6000型全自动凝血分析仪测定患儿脐血血浆D-D含量;法国梅里埃全自动荧光免疫分析仪(VIDAS)测定患儿脐血血浆PCT含量,比较两组D-D含量及其与窒息程度的关联性;同时选择40例正常新生儿作为对照组并测定相关数据,将各组数据进行对比研究。结果重度窒息组、轻度窒息组脐血血浆D-D、PCT含量较对照组均明显升高(P<0.05);重度窒息组脐血血浆D-D、PCT含量较轻度窒息组显著升高(P<0.05)。结论窒息患儿体内均存在不同程度的纤溶亢进,PCT含量明显升高,提示早产儿尤其是重度窒息患儿均存在不同程度的细菌感染,且窒息程度越重,感染现象越明显。Objective To investigate clinical value of umbilical cord blood plasma D-dimer(D-D) and procalcitonin(PCT) detection in asphyxia infant. Methods There were 40 mild asphyxia infants(mild asphyxia group) and 30 severe asphyxia infants(severe asphyxia group). Sysmex6000 automated coagulation analyzer was applied to detect plasma D-D content, and French bio Merieux automated fluorescent immunoassay analyzer(VIDAS) was used to detect plasma PCT content. Correlation of D-D content and asphyxia degree was compared between the two groups. Another 40 normal newborn was taken as control group, and their related data were used in comparative research. Results Both the severe and mild asphyxia groups had much higher plasma D-D and PCT contents than the control group(P〈0.05). The severe asphyxia group had obviously higher plasma D-D and PCT contents than the mild asphyxia group(P〈0.05). Conclusion Hyperfibrinolysis is shown by different degree in asphyxia infants. Obvious increase of PCT content reveals different degree of bacterial infection in premature infants, especially severe asphyxia infants. Severe asphyxia degree will lead to obvious infection.
