不同治疗方法对会阴创口愈合不良的效果分析

摘　　要：目的探讨高锰酸钾坐浴联合百克瑞与传统方法治疗会阴创口愈合不良的效果。方法 2013年6月至2015年6月温州市医科大学附属第三人民医院收治的孕妇5 820例,其中选择会阴侧切创口愈合不良患者92例为研究对象,采用随机对照原则将病人分为实验组和对照组,每组46例。实验组采用高锰酸钾坐浴联合百瑞克治疗,对照组用传统治疗方法,包括静脉或肌肉注射抗生素、清创换药、红外线照射、微波照射等。对比两组患者会阴创口疼痛等级、愈合程度、住院时间及费用。结果两组治疗后会阴创口疼痛等级均显著降低,差异有统计学意义(Z值分别为4.390、4.102,均P<0.01);实验组疼痛等级改善程度优于对照组,差异有统计学意义(Z=3.118,P<0.01);两组治疗后会阴创口愈合等级均显著改善,差异有统计学意义(Z值分别为8.391、7.790,均P<0.01);两组治疗后创口愈合程度组间对比,差异无显著性意义(Z=1.293,P>0.05);实验组住院时间、住院费用均显著少于对照组,差异有统计学意义(t值分别为10.207、13.983,均P<0.01)。结论高锰酸钾坐浴联合百克瑞治疗会阴创口愈合不良,能有效减轻患者痛苦,缩短住院时间,减轻患者经济负担。Objective To investigate the effect of potassium permanganate hip bath combined with Baikerui as well as conventional methods on poor healing of perineal wound. Methods From June 2013 to June 2015 totally 5 820 pregnant women were admitted in the Third People g Hospital of Wenzhou Medical University, of whom 92 cases with poor healing of perineal wound were selected. The cases were divided into experiment group and control group by random control principle with 46 cases in each. The experiment group accepted potassium permanganate hip bath combined with Baikerui, while the control group adopted conventional methods, including venous or intramuscular injection of antibiotics, surgical cleaning and dressing, infrared radiation, microwave radiation, etc. Two groups were compared in perineal wound pain level, healing degree, hospitalization length and expenses. Results After treatment the perineal pain level of two groups was alleviated remarkably, and the difference was statistically significant （Z value was 4. 390 and 4. 102, respectively, both P 〈0. 01 ）. The degree of pain relief in the experiment group was larger than that in the control group with statistically significant difference （Z = 3.118, P 〈 0. 01 ）. Perineal wound healing was significantly improved after treatment in two groups （ Z value was 8. 391 and 7. 790, respectively, both P 〈 0. 01 ）, but the difference between two groups was not statistically significant （Z = 1. 293 ,P 〉 0.05 ）. The hospitalization length and expenses of the experiment group were significantly shorter and less than those in the control group （t value was 10.207 and 13. 983, respectively, both P 〈 0.01 ）. Conclusion Potassium permanganate hip bath combined with Baikerui for treatment of perineal wound healing can effectively alleviate patients＇ pain, shorten the length of hospitalization, and reduce the economic burden for patients.
