益肾泄浊方联合前列地尔治疗慢性肾衰竭疗效观察

摘　　要：目的观察益肾泄浊方联合前列地尔治疗脾肾气(阳)虚兼瘀浊内阻型慢性肾衰竭患者的临床疗效及安全性。方法将90例脾肾气(阳)虚兼瘀浊内阻型慢性肾衰竭患者随机分为3组,中药组30例给予益肾泄浊方治疗,西药组30例给予前列地尔治疗,中西药组30例给予益肾泄浊方联合前列地尔治疗,3组疗程均为2周,观察3组治疗前后肾功能变化和疾病疗效、中医证候疗效及不良反应发生情况。结果 3组治疗后肾功能均较治疗前明显改善(P均<0.05),治疗后组间比较差异均无统计学意义(P均>0.05);3组治疗后疾病总有效率比较差异均无统计学意义(P均>0.05);中药组和中西药组中医证候疗效总有效率明显高于西药组(P均<0.05),差异有统计学意义(P均<0.05),中药组和中西药组比较差异无统计学意义(P>0.05)。结论益肾泄浊方、前列地尔及益肾泄浊方联合前列地尔均能改善脾肾气(阳)虚兼瘀浊内阻型慢性肾衰竭患者的肾功能,单用益肾泄浊方及联合使用前列地尔能明显改善患者的临床症状,提高患者的生活质量。Objective It is to observe the clinical efficacy and safety of Yishen Xiezhuo decoction combined with Alprostadil in the treatment of chronic renal failure of Qi（ Yang） deficiency of spleen and kidney with blood and turbid phlegm stasis syndrome. Methods Ninety patients with Qi（ Yang） deficiency of spleen and kidney with blood and turbid phlegm stasis syndrome were randomly divided into three groups： Chinese medicine group（ n = 30） treated with Yishen Xiezhuo decoction,western medicine group treated with Alprostadil,and combination group treated with Yishen Xiezhuo decoction combined with Alprostadil. All the groups were treated for 3 weeks. The changes of renal function before and after treatment,curative effect of disease and traditional Chinese medicine syndrome were observed in the three groups. Results The renal function were significantly improved after treatment compared with that before treatment in the three groups（ P〈0. 05）,but there was no statistical difference among the three groups（ P〉0. 05）. There was no significant difference in disease total effective rate among the three groups（ P〉0. 05）,but the syndrome effective rate in Chinese medicine group and combination group was significantly higher than that in western medicine group（ P〈0. 05）,there was no significant difference in the rate between Chinese medicine group and combination group. Conclusion Yishen Xiezhuo decoction,Alprostadil and combination of the drugs all can improve the clinical symptoms and renal function of patients suffering from chronic renal failure of Qi（ Yang） deficiency of spleen and kidney with blood and turbid phlegm stasis syndrome,and the effect of simple Yishen Xiezhuo decoction or combination with Alprostadil is better.
