血清APE1自身抗体在结直肠癌诊断中的价值

摘　　要：目的探讨血清中APE1自身抗体(apurinic/apyrimidinic endonuclease 1 autoantibodies,APE1-AAbs)在结直肠癌中的诊断价值。方法利用ELISA检测150例结直肠癌患者和170例健康体检人员(对照组)血清APE1-AAbs水平,采用蛋白芯片法检测相应的CEA、CA199和CA242浓度。运用Logistic回归和ROC曲线分析方法,将APE1-AAbs与CEA、CA199及CA242联合分析。结果 150例结直肠患者血清APE1-AAbs中位水平为2.697,显著高于对照组(P<0.001)。绘制ROC曲线,其AUC为0.797,灵敏度为62.67%,特异性为85.29%。APE1-AAbs分别与CEA、CA199及CA242联合后,AUC、诊断灵敏度和准确性均有所升高。结论血清APE1-AAbs对结直肠癌有辅助诊断价值,与临床上常用的结直肠癌肿瘤标志物联用可提高结直肠癌的诊出率。Objective To explore the diagnostic value of serum apurinic /apyrimidinic endonuclease 1autoantibodies（ APE1-AAbs） in screening colorectal cancer. Methods Serum APE1-AAbs of 150 patients with colorectal cancer and 170 healthy controls were detected by ELISA. Carcinoembryonic antigen（ CEA）,carbohydrate antigen 19-9（ CA199） and carbohydrate antigen 242（ CA242） were detected by protein chip technology. Logistic regression and receiver operative characteristic（ ROC） curve were employed for combined analysis of APE1-AAbs with CEA,CA199 or CA242. Results The median level of serum APE1-AAbs in the patients with colorectal cancer was 2. 697, which was higher than that of the healthy controls（ P 〈0. 001）. With the ROC curve,area under curve（ AUC） was 0. 797,and the sensitivity and specificity were 62. 67% and 85. 29%,respectively. The AUC,diagnosis sensitivity and accuracy of colorectal cancer were increased when combining serum APE1-AAbs with CEA,CA199 or CA242. Conclusion Serum APE1-AAbs can help to diagnose colorectal cancer,and its combination with CEA,CA199 or CA242 can improve the early diagnosis accuracy of colorectal cancer.
