摘 要:目的:探讨远隔缺血后处理与右美托咪啶联用对老龄大鼠脑缺血再灌注损伤时TNF—α、ICAM-1的表达及术后认知功能的影响。方法:选择健康SD老龄大鼠60只(月龄为18-20月),体质量为280-320g,将其随机分为5组(n=12),即假手术组(S组)、脑缺血再灌注组(I/R组)、远隔缺血后处理组(R组)、右美托咪啶组(D组)、远隔缺血后处理联合右美托咪啶组(R+D组);采用线栓法制备大鼠局灶性脑缺血再灌注损伤模型(MCAO);D组大鼠在缺血即刻静脉注射右美托咪啶3pg/kg,随后以3μg/(kg·h)速率输注120min;R组大鼠夹闭右下肢股动脉阻断血流5min,恢复5min,重复3次;R+D组大鼠在给予右关托咪啶同时夹闭右下肢股动脉阻断血流5min,恢复5min,重复3次;S组和I/R组、R组再灌注恢复前给予等容量0.9%氯化钠注射液;再灌注24,48和144h,每组随机取2只大鼠处死,取大脑组织,采用放射免疫法(RIA)测定TNF-α含量和免疫组织化学染色检测ICAM-1的表达;其余大鼠行Morris水迷宫实验,测定其认知功能。结果:与I/R组比较,第1~5天R组、D组、R+D组大鼠逃避潜伏期明显缩短(P〈0.05);与D组和R组比较,第2~5天R+D组大鼠逃避潜伏期缩短(P〈0.05),穿越平台次数增多;与I/R组大鼠比较,R组、D组和R+D组大鼠各时点脑组织TNF—α和ICAM—1的表达水平下调;与R组和D组比较,R+D组各时点脑组织TNF—α和ICAM-1的表达水平下调更明显。结论:远隔缺血后处理与右美托咪啶联用可抑制脑损伤后细胞因子TNF—α和ICAM-1的表达,从而改善老龄大鼠术后认知功能。Objective: To explore the influence of cognitive function of dexmedetomidine combined with ischemic postconditioning on aged rats with cerebral ischemia in vitro. Methods: 60 healthy male aged SD rats(18 to 20 months of age and Mass of rats were between 280 and 320 g) were randomly divided into 5 groups(n=12): sham operation group (S group),ischemia and reperfusion group (group I/R), remote ischemic post treatment group (Group R), dexmedetomidine group (group D) and remote ischemic postprocessing combined with dexmedetomidine group (Group R+D). Rat focal cerebral ischemia reperfusion injury model was made by suture method (MCAO). Group D in ischemia immediate intravenous dexmedetomidine set 3 μg/kg, followed by 3μg/(kg. h) rate infusion 120min, R group separates and blocks the right femoral artery for 5 min and 5 min recovery, giving on three ischemia/re perfusion, D-R group in given dexmedetomidine separates and blocks the right femoral artery blood flow for 5 min, restores 5 rain and repeats 3 times of ischemia/reperfusion cycles. S group, I/R group and R group reperfusion before the resumption was given a volume of physiological saline. Reperfusion 24 h, 48 h, 144 h for every randomly selected two rats was from brain tissue, the content of TNF-α was measured by radioimmunoassay(RIA), and immunohistochemical staining detection of ICAM-1 expression; more than rats received morris water maze test and determinated of cognitive function. Results: Compared with group I/R, the first and the fifth days R group, D group and R+D group rats escaped latency, significantly shorter (P〈0.05). Compared with group I/R, the third and the fifth days R group and D group, R/D group, escaped laten, the second and the fifth days R/D group significantly shorter escaped latency (P〈0.05).
