调肝汤治疗肾性贫血的临床研究

摘　　要：目的:观察调肝汤对肾性贫血的临床疗效。方法:将92例肾性贫血患者随机分为试验组和对照组各46例,试验组予调肝汤每日1剂,每日分2次服用;对照组予尿毒清颗粒每次1袋,每日4次服用;两组均给予慢性肾衰竭一般治疗及肾性贫血常规治疗。总疗程8周。两组治疗前后分别观察血常规、铁代谢指标、肾功能、计算内生肌酐清除率、使用重组人红细胞生成素总量及证候疗效。结果:两组治疗后血常规指标均较治疗前升高(P<0.01),试验组治疗后RBC、Hb、HCT均高于对照组,差异有统计学意义(P<0.05);试验组治疗后SF水平高于对照组(P<0.05);两组治疗后TS比较差异无统计学意义(P>0.05);两组患者治疗后肾功能均有改善(P<0.01),两组治疗后肾功能各指标组间比较均无统计学意义(P>0.05);试验组使用EPO总量明显少于对照组(P<0.01);两组证候疗效总有效率差异无统计学意义(P>0.05),试验组和对照组愈显率分别为47.83%和15.21%,差异有显著统计学意义(P<0.01)。结论:中药调肝汤治疗肾性贫血疗效确切,并且能改善肾功能和临床证候,减少EPO用量。Objective： To observe the therapeutic effect of Tiaogan Decoction for renal anemia. Methods： Ninety- two cases of renal anemia patients were randomized into the treatment group（ 46 cases） and control group（ 46 cases）. The treatment group was given Tiaogan Decoction,twice a day,1/2 dose per each time while the control group was given Niaoduqing Granules,four times a day,1 bag per each time. All patients were given general treatment for chronic renal failure and renal anemia. All cases were treated for 8 weeks. The routine blood test,iron metabolism indexes,renal function,creatinine clearance rate（ Ccr）,the total application amount of EPO and syndrome effect were detected before and after treatment. Results： Theroutine blood test indexes of both groups after treatment were ameliorated（ P 0. 01）. The values of RBC,Hb and HCT of treatment group were higher than the control group＇s after treatmentwith a significant difference between groups（ P 0. 05）. The level of SF of treatment group was higher than that of the control group（ P 0. 05）. There was no significant difference between the TS levels of the two groups after treatment（ P 0. 05）. Renal functions of both two groups were ameliorated after treatment（ P 0. 01）. There were no significant differences of renal function indexes between groups after treatment（ P 0. 05）. The total amount of EPO in the treatment group was significantly less than that of the control group（ P 0. 01）. There was no significant difference between total effective rates of the two groups（ P 0. 05）. The excellent rate of the treatment group and the control group was 47. 83% and 15. 21% respectively,with a significant difference between groups（ P 0. 01）. Conclusion： Tiaogan Decoction is effective in treating renal anemia,improving renal function,ameliorating clinical syndrome and reducing the application amount of EPO.
