痹痛灵冲剂治疗类风湿关节炎寒湿阻络证及对炎性细胞因子的作用研究

摘　　要：目的:分析痹通灵冲剂在证属寒湿阻络的类风湿关节炎(RA)治疗中的应用价值,为通过临床治疗效果提供参考。方法:应用随机数字表法将80例证属寒湿阻络的RA患者分为两组,每组40例。对照组接受艾甲氨蝶呤联合双醋瑞因治疗,治疗组接受甲氨蝶呤、双醋瑞因联合痹通灵冲剂治疗。对比两组临床治疗效果及对炎性细胞因子变化情况。结果:入组时两组间晨僵时间及RA活动性评分(DAS28)比较未见统计学差异(P>0.05),而在完成2个月的治疗时,治疗组晨僵时间及DAS28评分均低于对照组(P<0.05)。同时两组间治疗总有效率比较未见统计学差异(P>0.05)。而入组时两组间类风湿因子(RF)、C反应蛋白(CRP)及血沉(ESR)比较未见统计学差异(P>0.05),而在完成2个月的治疗时,治疗组RF、CRP及ESR水平均低于对照组(P<0.05)。此外,治疗组治疗不良反应发生率明显低于对照组(P>0.05)。结论:在证属寒湿阻络的RA的治疗中,痹通灵冲剂可在不降低治疗效果的同时,有效的改善患者的炎性细胞因子及药物不良反应发生率。Objective：To analyze the impact inflammatory cytokines of Bitongling infusion in rheumatoid arthritis genus cold dampness resistance winding,and provide reference for through clinical therapeutic effect.Methods：Applied random number table method,80 example of cold dampness resistance winding RA patients were divided into two groups,40 cases in each group.Control group accepted Iguratimod combine diacerein treatment,treatment group received Iguratimod,diacerein combine Bitongling infusion treatment.Compared two groups of clinical curative effect and changes of inflammatory cytokines.Results：There was no statistical difference of morning stiffness time and RA activity score（DAS28）between the groups（P〉0.05）,and after 2months of treatment,the morning stiffness time and DAS28 scores of the treatment group were lower than that of control group（P〈0.05）.At the same time,there was no statistical difference of total effective rate between the two groups（P〉0.05）.There was no statistical difference of rheumatoid factor（RF）,c-reactive protein（CRP）and blood sedimentation（ESR）between the two groups（P〈0.05）,but after 2months of treatment,the RF,ESR and CRP of treatment group were lower than the control group（P〈0.05）.The incidence of adverse reactions to treatment in addition,the treatment group was obviously lower than the control group（P〈0.05）.Conclusion：In the genus cold dampness resistance winding the treatment of RA,the psychic granule can reduce treatment effect without certification of at the same time,the effective improvement in patients with inflammatory cytokines and the incidence of adverse drug reactions.
