富含血小板血浆治疗慢性骨髓炎不愈合创面临床研究

摘　　要：目的:探讨富血小板血浆(platelet-rich plasma,PRP)对慢性骨髓炎不愈合创面临床治疗效果。方法:收集2010年3月—2013年3月我院收住慢性骨髓炎合并创面不愈合病例20例,采用局部PRP注射结合常规疗法,作为治疗组(20例),其中胫骨8例,踝关节3例,股骨5例,跟骨3例,肱骨1例。回顾调查同期我院采取常规疗法治疗慢性骨髓炎合并创面不愈合病例20例,作为对照组,其中胫骨7例,踝关节2例,股骨7例,跟骨2例,肱骨2例。两组病例均排除糖尿病、血液系统疾病、免疫疾病、结缔组织病等影响伤口愈合的基础疾病。各组年龄及性别等资料差异无统计学意义(P>0.05)。观察创面愈合情况、治疗时间、治疗费用指标,研究PRP对慢性骨髓炎合并创面不愈合的治疗作用及对预后的影响。结果:所有病例均获得随访,随访时间6个月到2年。其中治疗组20例,创面愈合情况优15例,良4例,差1例,优良率为95.0%;治疗费用平均为(2.48±0.78)万元;住院时间平均(27±13.01)天;对照组20例,创面愈合情况优10例,良3例,中2例,差5例,优良率为65.0%;治疗费用平均为(3.25±1.21)万元;住院时间平均(35±12.83)天。结论:应用富含血小板血浆(PRP)治疗慢性骨髓炎不愈合创面比较传统治疗对照组,具有较高的治愈率(P<0.05),治疗费用低(P<0.05),住院周期短(P<0.05)。PRP疗法在慢性骨髓炎创面不愈合治疗领域是一种全新的治疗理念和治疗方法。Objective：Discuss the clinical treatment of chronic osteomyelitis＇nonunion wound by Plateletrich plasma （PRP）. Methods： Collected 20 cases of chronic osteomyelitis patients with nonunion wound during March 2010 to March 2013 in our hospital, used PRP local injection with routine treatment. As the treatment group （20 cases）, there were 8 cases of tibia, 3 cases of ankle , 5 cases of femur, 3 cases of calcaneus and 1 case of humerus. We collected 20 cases of chronic osteomyelitis patients with nonunion wound at same period as the control group which only use routine treatment, there were 7 cases of tibia, 2 cases of ankle , 7 cases of femur, 2 cases of calcaneus and 2 cases of humerus. Both groups were excluded diabetes, blood system diseases, immunological diseases, connective tissue disease desmosis and other diseases that influence wound healing. There was no significant difference in age and gender （P 〉0.05）. We chose intertion status, time and cost of therapy as our observation index. All datum were processed by statistical software （SPSS17.0）. Comparing the differences between the two groups to research the treatment and prognosis of PRP in chronic osteomyelitis＇ nonunion wound. Results：All patients were followed up for 6 months to 2 years. The 20 cases of the treatment group were consists of 13 male cases and 7 female cases, with an average age of 39.2 ±4.5. Wound healing was very well in 15 cases, well in 4 cases, and poor in 1 case, the very well and well rate was 95 %. The average cost of treatment was 24.80 ± 7. 80 thousand. The length of stay was 27 ± 13.01 days. The 20 cases of the control group were consists of 14 male cases and 6 female cases, with an average age of 38.5 ± 3.9. Wound healing was very well in 10 cases, well in 3 cases, medium in 2 cases, and poor in 5 cases, the very well and well rate was 65%. The verage cost of treatment was 32. 50 ± 12.10 thousand.
