老年高血压患者脉压与心率减速力相关分析

摘　　要：目的:探讨老年高血压患者脉压(pp)与心率减速力关系。方法:对150例老年高血压患者进行动态血压检测,根据脉压(pp)分为三组,A组60例(pp<40mm Hg),B组47例(40mm Hg≤pp≤60mm Hg),C组43例(pp>60mm Hg),所有患者行动态心电图,离线分析后计算出心率减速力(DC值)、心率变异性(HRV)时域指标,三组间进行比较分析。结果:C组与A组比较DC值、NN间期标准差(SDNN)、NN间期平均值的标准(SDANN)、相邻NN间期差值的均方差(r MSSD)均显著降低(P<0.05),C组与B组比较DC值、SDNN、SDANN、r MSSD均降低(P<0.05),B组与A组比较DC值、SDNN、SDANN、r MSSD均降低(P<0.05)。结论:老年高血压脉压与DC值及HRV呈负相关,对高血压患者不仅降低血压,更要缩小脉压,减小自主神经功能损害。Objective：To investigate the Relationship between pulse pressure（pp） and deceleration capacity of heart rate in elderly patients with Hypertension. Methods： 150 cases of elderly patients with Hypertension were selected to detect the ambulatory blood pressure and then according to the PP, the patients were divided into three groups： 60 cases of Group A （pp 〈40 mmHg） ; 47 cases of Group B （40mmHg 〈 pp 〈60mmHg） ;43 cases of Group C （pp 〉60mmHg）. All patients were examined the ambulatory electrocardiogram, and then carried out the off- line analysis to calculate the DC value and HRV time domain index. The results were analyzed comparatively among the three groups. Results：Compared with group A, the values of DC ,SDNN,SDANN and rMSSD were sig- nificantly lower （P 〈 0.05 ） in group C ; whereas, compared with group B, the DC value, SDNN, SDANN and rMSSD were also lower （P 〈 0.05 ） in group C; compared with group A, the DC value, SDNN, SDANN and rMSSD were lower （P 〈 0.05 ） in group B. Conclusion：Elderly patients with hypertension, the pulse pressure is negatively correlated with HRV and DC. It is suggested that not only the blood pressure in patients with hypertension should be lowed but also should reduce the pulse pressure difference, so the damage of autonomic nerve function can be prevented in some degree.
