摘 要:目的研究血浆内皮素-1水平与高血压的关系,探讨高血压患者血浆内皮素-1水平对血压变异的影响,阐明高血压患者血浆内皮素-1水平与血压及血压变异的关系。方法 2012年1月—2013年12月筛选104例高血压患者作为高血压组,并根据血压数值及靶器官损害情况分为高血压1级组36例、高血压2级组36例和高血压3级组32例,筛选同期健康体检者45例作为对照组。所有对象均进行无创性24 h动态血压监测,次日清晨空腹抽取肘静脉血,测定血浆内皮素-1水平。计量资料采用t检验或单因素方差分析,两变量间相关性分析采用Pearson线性相关,多因素分析采用多元线性回归分析,P〈0.05为差异有统计学意义。结果高血压1级组、高血压2级组和高血压3级组血浆内皮素-1水平分别为(2.66±0.97)、(3.93±1.77)、(5.79±2.31)ng/ml,均明显高于对照组的(1.31±0.11)ng/ml,差异均有统计学意义(均P〈0.05)。血浆内皮素-1水平与24 h平均收缩压、24 h平均舒张压均呈显著正相关(r=0.637、0.879,均P〈0.05)。血浆内皮素-1水平与24 h收缩压变异系数、24 h舒张压变异系数呈显著正相关(r=0.863、0.633,均P〈0.05)。多元逐步回归分析表明,24 h平均收缩压(OR=1.47,95%CI为1.03~2.13)、24 h平均舒张压(OR=2.13,95%CI为1.16~3.98)、24 h收缩压变异系数(OR=2.44,95%CI为1.32~4.76)及24 h舒张压变异系数(OR=0.47,95%CI为0.24~0.85)是内皮素-1水平的影响因素。结论进一步证实了血浆内皮素-1的水平不仅与血压升高有关,而且与血压变异性关系密切。据此我们推论血浆内皮素-1可能参与高血压的发生、发展机制,血压水平及血压变异是导致内皮素-1水平升高的危险因素。Objective To investigate the relationship between ET-1 with hypertension and blood pressure variability.Methods104 patients with hypertension and 45 normal controls from January 2012 to December 2013 were enrolled into this study.The104 patients were diagnosed according to the international standard,the 45 normal controls were selected from the physical examination center,hypertension group was divided into three experimental groups according to the value of blood pressure and target organ damage.All objects were monitored 24 h ambulatory blood pressure,plasma levels of ET-1 was measured in the next morning.Measurement data was processed by t test and ANOVA,correlation analysis of the two variables used Pearson correlation,multiple factors analysis used multiple linear regression analysis,P0.05 was considered statistically significant.Results The level of plasma ET-1 of hypertensive level 1 group,hypertensive level 2 group,hypertensive level 3 group were(2.66±0.97),(3.93±1.77)(5.79±2.31)ng/ml,higher than that of the control group[(1.31±0.11)ng/ml],the differences were statistically significant(all P0.05).Correlation analysis showed that the level of ET-1 was significantly positively correlated with 24 h systolic blood pressure and 24 h diastolic blood pressure(r=0.637,0.879,all P0.05).The level of ET-1 was significantly positively correlated with 24 h systolic blood pressure variability and 24 h diastolic blood pressure variability(r=0.863,0.633,all P0.05).Multiple regression analysis showed that 24 h systolic(OR=1.47,95%CI 1.03-2.13),24 h diastolic(OR=2.13,95%CI 1.16-3.98),24 h systolic blood pressure variability(OR=2.44,95%CI 1.32-4.76) and 24 h diastolic blood pressure variability(OR=0.47,95%CI 0.24-0.85)were still significantly correlated with plasma ET-1.Conclusion ET-1 was significantly positively correlated with blood pressure and blood pressure variability,ET-1 involved in hypertension and blood pressure development.The level of blood pressure was the hazard
