腹腔镜与开腹直肠癌根治术临床对比观察

摘　　要：目的:比较腹腔镜下直肠癌根治术和开腹直肠癌根治术在直肠癌外科手术治疗中的优劣势。方法:回顾性分析2013年6月—2015年6月成功完成的直肠癌根治术77例患者临床资料,其中腹腔镜及开腹直肠癌根治术分别为36例及41例。对2组患者的切口长度、手术时间、术中出血量、淋巴结清扫数目、术后腹腔引流量、术后排气时间、术后并发症、保肛率和术后住院时间进行比较。结果:腹腔镜组的手术切口长度、手术时间、术中出血量、术后腹腔引流量、术后排气时间和术后住院时间均明显少于开腹组;2组的保肛率、病理检出淋巴结数目和术后并发症比较,差异无统计学意义。结论:腹腔镜直肠癌根治术和传统开腹手术都是切实可靠、术式安全的治疗方法,其相比传统开腹手术在淋巴结清扫以及术后并发症上无明显差异,但因其创伤性小、恢复快等优点,该术式已被广大医生所接受和使用。Objective： Comparison of laparoscopic colorectal cancer resection and open colorectal cancer resection in colorectal cancer surgery of the advantages and disadvantages. Methods： A retrospective analysis in June 2013-2015 completed successfully in June 77 cases of colorectal cancer resection patients with clinical data, including laparo- scopic and open radical resection of colorectal cancer were 36 cases and 41 cases. 2 groups of patients with incision length, operative time, blood loss, number of lymph node dissection, postoperative abdominal drainage, postoperative exhaust time, postoperative complications, the rate of sphincter preservation and postoperative hospital stay were com- pared. Results： The length of incision laparoscopic group, operative time, blood loss, postoperative abdominal drainage postoperative exhaust time and postoperative hospital stay were significantly less than the laparotomy group; sphincter preservation rate of 2 groups of pathological The numl6er of lymph nodes and detection of postoperative complications, the difference was not statistically significant. Conclusion： The laparoscopic colorectal cancer resection and traditional open surgery are practical and reliable, safe surgical treatment, which compared to traditional open surgery and lymph node dissection was no significant difference in postoperative complications, but because of trauma small, quick recov- ery, etc., the procedure has been accepted by the majority of doctors and use.
