中国农村高血压与心血管危险因素的防治不容乐观

摘　　要：目的研究中国农村高血压和心血管危险因素的现状和探索国产降压药物的治疗效果。方法采用多阶段整群抽样方法,从2003年10月-2005年5月之间,在河南信阳7个社区进行以社区为基础的横断面研究。总计调查40-75岁之间共15,830个成年人,对所有患者问卷调查及进行肝功能、肾功能、血脂、血糖及心电图等检查。同时给予部分高血压患者国产降压药物干预治疗4周,观察血压达标情况。结果信阳农村高血压患病率40.3%,知晓率44.2%,治疗率37.6%,治疗血压达标率非常低为6.2%。50%的患者携带1-2个危险因素,40%的患者携带3个或3个以上心脑血管危险因素。4种国产降压药物均能有效降低血压,其中伲福达(硝苯地平缓释片Ⅱ)单药治疗组血压达标率最佳。结论农村高血压负担沉重,4种国产降压药副作用比较小,降压效果可靠,能够担当农村地区降低血压的重任。Objective The purpose of the study was to investigate current situation of hypertension in rural areas and determine whether domestical antihypertensive drugs could get better blood pressure goal.Methods By using multistage clustering sampling method,a total of 15830 adults aged 40-75 years were enrolled.All patients were investigated by questionnaire and were given biochemical examination.Some patients with hypertension were treated with domestical antihypertensive drugs for 4 weeks.Results The prevalence,awareness and treatment,of hypertension in rural areas of Xinyang was 40.3%,44.2%,37.6%.respectively.The control of blood pressure was very low.only6.2%.50%of the patients carried 1-2 cardivascular risk factors,and 40%of the patients with 3 or more than 3 cardiovascular risk factors.Four domestical antihypertensive drugs could effectively lower blood pressure,which of the patients with nifedipine（nifedipine sustained release tablets II） monotherapy group got better blood pressure goal.Conclusions The burden of hypertension in rural areas were severe.Antihypertensive drugs made in china could play an important role in rural areas because of the better effects of lowering blood pressure and fewer side effects.
