某三级医院2012～2015年多重耐药菌变化趋势分析

摘　　要：目的掌握本院细菌耐药菌的状况,为目标性监测管理多重耐药菌提供依据。方法分析2012-01-01~2015-12-31本院临床检出病原菌的种类、数量、标本来源、多重耐药菌的占比及0~14 a儿童多重耐药菌的占比情况,以SPSS16.0软件进行数据分析。结果 2012~2015年度本院共检出多重耐药菌4 889株,其中0~14 a的儿童检出多重耐药菌3767株,占77.05%;多重耐药菌排序为前3位的是：1大肠埃希菌ESBLS 1 198株（24.50%）;2金黄色葡萄球菌MRSA 646株（13.21%）;3肺炎克雷伯菌ESBLS 427株（8.73%）。大肠埃希菌ESBLS检出率2012年为25.59%,2013年为28.03%,2014年为30.86%,2015年为30.31%。结论 2012~2015年大肠埃希菌ESBLS的检出率呈上升趋势;0~14 a儿童大肠埃希菌ESBLS感染的病例应引起高度重视,规范抗菌药物管理,预防耐药菌的产生;科学管理多重耐药菌感染的患者,预防医院感染的发生。OBJECTIVE To provide the basis for targeted monitoring and management of multi-drug resistant organisms（ MDRO）by analyzing the antibiotic resistance of bacteria in the hospital. METHODS The species,quantity,the proportion of the MDROs of the bacteria and types of sample from patients,and the proportion of the MDROs among children aged from 0 to 14 from January 1,2012 to December 31,2015 were analyzed by SPSS16. 0. RESULTS A total of 4 889 strains of MDRO were cultured from patients,in which 3767（ 77.05%） strains were from children aged from 0 to 14,from 2012 to 2015. The first three MDROs were： 1Escherichia coli（ ESBLS）,1198 strains（ 24. 50%）; 2 staphylococcus aureu（ MRSA）,646 strains（ 13. 21%）; 3 Klebsiella pneumoniae（ ESBLS）,427 strains（ 8.73%）. The rates of Escherichia coli（ ESBLS） was 25.59% in 2012,28.03% in 2013,30.86% in 2014 and30.31% in 2015,respectively. CONCLUSION The rates of Escherichia coli（ ESBLS） showed an upward trend. The cases of children aged from 0 to 14 years infected by Escherichia coli（ ESBLS） should be taken seriously. The management of antimicrobial drugs should be regulated to prevent the emergence of MDRO,and to prevent the healthcare-associated infection in hospital.
