摘  要:目的:通过CT冠状动脉造影(CTCA)扫描方案的改良以控制对比剂剂量,评价图像质量与辐射剂量,明确该改良方案的临床应用价值。方法患者70例,随机分为2组。A组采用手动触发扫描,CTCA扫描注射10~12 s对比剂药量,B组采用循环时间测试扫描,循环时间测试对比剂剂量20 mL ,CTCA扫描注射14 s对比剂药量。比较2组图像质量、对比剂剂量与辐射剂量。统计学方法采用两独立样本 t检验和χ2检验,检验水准α=0.05。结果 A、B 2组主观图像质量差异无统计学意义,客观图像质量差异无统计学意义(P>0.05)。A组对比剂剂量少于B组,差异具有统计学意义(48.14 mL ±3.85 mL vs 86.60 mL ±6.21 mL , t=-31.142,P=0.000)。A、B 2组辐射剂量差异无统计学意义(4.94 mSv vs 5.66 mSv ,t=-1.475,P=0.145)。结论手动触发扫描可以在大幅度减低对比剂剂量的前提下获得可评估的图像质量。Objective To compare the image quality and radiation dose between bolus tracking CTCA and time bolus CTCA pro‐tocols .Methods 70 consecutive patients with suspected CAD were prospectively enrolled .The patients were divided into two groups according to manual trigger bolus or test bolus .The amount of contrast medium ,radiation dose and image quality were compared be‐tween two groups .Unpaired t‐test and chi‐square test were used in the study .Results The differences in the attenuation ,noise , SNR and CNR of the coronary arteries between two groups were not statistically significant (P〉0 .05) .And the difference in the mean image quality scores between A and B groups (3 .65 ± 0 .59 vs 3 .66 ± 0 .55) was not statistically significant (t= -0 .161 ,P=0 .904) .However ,the mean contrast media dose in group A was lower than that in group B (48 .14 mL ± 3 .85 mL vs 86 .60 mL ± 6 .21 mL ,t= -31 .142 ,P=0 .000) .The difference in mean ED was not statistically significant between both groups (4 .94 mSv vs 5 .66 mSv ,t= -1 .475 ,P=0 .145 ) .Conclusion Bolus tracking CTCA with low contrast media dose could offer assessable image quality in patients .
