摘 要:目的:探讨CTU在上尿路梗阻性疾病中的诊断价值。方法:对112例上尿路梗阻性疾病的患者行CTU检查,对所获容积数据进行最大密度投影、多平面重建、容积再现和曲面重建三维后处理,然后分析其影像表现及诊断结果,并与IVP、MRU进行比较,对照手术病理结果进行分析。结果:112例中CTU确诊108例,误诊1例,3例无法诊断,确诊率为96.4%,明显高于IVP(82.3%)和MRU(89.3%),差异均有统计学意义(P<0.05)。CTU对上尿路梗阻性疾病定位诊断准确率为100%。结论:CTU完整清晰地显示了上尿路结构及导致其梗阻的病变及其形态,提高了上尿路梗阻性疾病的定位和定性诊断率,为临床保守或手术治疗提供可靠依据,可作为上尿路梗阻性疾病患者的首选影像学检查方法。Objective:To explore the value of CTU in diagnosis of upper urinary tract obstruction.Method:112 cases of upper urinary tract obstruction diseases underwent CTU examination,the volume data of all patients were reconstructed with 3D reconstruction technigues included maximum intensity projection, multipie plane reformation,and volume rendering technigue,and curve plane reformation,and compared with IVP and MRU, the results of surgery and pathology were analyzed.Result:The diagnostic accuracy of CTU was 96.4%,significantly higher than IVP(82.2%) and MRU(89.3%),the differences were statistically significant(P<0.05).The accuracy of CTU in the diagnosis of upper urinary tract obstruction was 100%.Conclusion:CTU completely and clearly shows the structure of the upper urinary tract and lead to the obstruction of the lesions and its shape,increase the rate of upper urinary tract obstruction diseases of localization and qualitative diagnosis,and provide a reliable basis for clinical conservative treatment or operation,as in patients with upper urinary tract obstruction disease the first choice of imaging examination method.
