四级早期活动与康复锻炼疗法预防病人ICU获得性衰弱的效果观察

摘　　要：[目的]探讨四级早期活动与康复锻炼疗法预防病人重症监护室获得性衰弱(ICU-AW)的效果。[方法]选取综合ICU收治并符合纳入标准的重症病人100例随机分为干预组和对照组,对照组按照ICU护理常规给予四肢被动活动和功能锻炼,干预组在对照组基础上采用四级早期活动与康复锻炼疗法,干预后比较两组病人的肌力、巴氏指数、机械通气时间、ICU住院时间、总住院时间、呼吸机相关性肺炎(VAP)发生率、深静脉血栓发生率、压疮发生率和ICU-AW发生率。[结果]干预后,干预组病人的肌力、巴氏指数评分均高于对照组(P<0.01),机械通气时间、ICU住院时间和总住院时间少于对照组(P<0.01),VAP发生率、压疮发生率和ICU-AW发生率低于对照组(P<0.01)。[结论]采用四级早期活动与康复锻炼疗法可提高病人的肌力和自理能力,减少ICU-AW的发生,缩短住院时间。Objective:To probe into the effect of four stage early activity and rehabilitation exercise therapy for prevention of patients with ICU-AW.Methods:A total of 100 critically ill patients who were received and cured in comprehensive ICU and met the inclusion standards were randomly divided into intervention group and control group,the patients in control group were given the four limbs passive activity and functional exercise according to the ICU nursing routine,while the patients in intervention group received the four stage early activity and rehabilitation exercise therapy on the basis of the method in the control group.After the intervention,compared the muscle strength,Barthel index,mechanical ventilation time,ICU hospitalization time,total hospitalization time,ventilator-associated pneumonia(VAP)incidence rate,deep vein thrombosis incidence rate,pressure sore incidence rate and ICU-AW incidence rate of patients in two groups.Results:After the intervention,the patients' muscle strength and Barthel index in intervention group were higher than those of the control group(P<0.01);mechanical ventilation time,ICU hospitalization time and total hospitalization time were less than those of the control group(P<0.01);the VAP incidence rate,pressure sore incidence rate and ICU-AW incidence rate were lower than those in the control group(P<0.01).Conclusion:The application of four stage early activity and rehabilitation exercise therapy can improve the patient's muscle strength and self-care ability,reduce the incidence of ICU-AW and shorten the hospitalization time.
