结直肠癌术后腹泻患者肛周皮肤保护方法的比较研究

摘　　要：目的:探讨结直肠癌术后腹泻患者保护肛周皮肤较好的方法或方案。方法:将140名结肠癌和直肠癌术后腹泻患者随机分为4组:造口粉组、3M皮肤保护膜(简称保护膜)组、集便袋组和保护膜+集便袋组,持续观察5 d,比较其肛周皮肤保护效果(是否出现肛周皮肤红肿、糜烂或破损),以及所需护理时间。结果:造口粉组、保护膜组、集便袋组和保护膜+集便袋组等对术后肛周皮肤的保护作用差异有显著性,P<0.05;其中保护膜组、集便袋组和保护膜+集便袋组等3组之间差异无显著性,P>0.05;造口粉组对术后的肛周皮肤保护作用较其他组别低,P<0.05。造口粉组每日肛周皮肤护理时间均显著长于其他各组,P<0.05;保护膜组的护理时间也显著长于集便袋组和保护膜+集便袋组,P<0.05。结论:保护膜、集便袋和保护膜+集便袋均可以有效保护结直肠癌术后患者的肛周皮肤,保护膜+集便袋还可以减少护理的工作量。Objective： To investigate the approaches to care of the perianal skins in patients with diarrhea after surgery for colorectal cancer. Methods： 140 patients undergone surgery for colorectal cancer were randomized into 4 group as skin care with stoma powder,3-m skin protective film,waste collecting bag and protective film plus collecting bag. After observation for consecutive 5 days,the protective effects were compared,including presence of the swelling,erosion or damage to the perianal skins as well as length of the care. Results： Protective effects were significantly different among the four groups of patients received diverse skin care（ P〈0. 05）,however,the difference was not significant in patients cared protective film,collecting bag and protective film plus collecting bag（ P〈0. 05）. Perianal skin care with stoma powder had lower protective effect and longer daily care（ P〈0. 05）. Perianal skin care with simple protective film consumed relatively longer time than care with collecting bag and combined protective film with collecting bag（ P〈0. 05）. Conclusion：Although the four approaches can contribute to protection of the perianal skins in patients with diarrhea after surgery for colorectal cancer,yet combined protective film with collecting bag can achieve better effect and relieve the burden of nurses in their daily work.
