摘  要:目的探究3D显示技术在组织肿瘤细胞观察及诊断中的适用性及可行性。方法成像效果评估:安排10名病理医师先后以数字显微镜3D系统、2D图像系统及普通光学显微镜对30份病理切片进行观察,对比3D和2D系统在成像质量上的差异。临床应用测试:收集甲乳外科术中冰冻切片40份,安排1名病理科医师在3D及2D系统下得出每个切片对应诊断,对比3D和2D图像系统在冰冻切片肿瘤细胞诊断准确性上的差异。结果视觉效果评估结果显示,3D及2D系统在标本边缘清晰度、色彩还原度、视野范围、视野内均匀成像、视野亮度、视野内噪点、移动视野延时感、标本形状无变异方面差异无统计学意义(P>0.05),而在深度感/立体感、视觉调节方面差异有统计学意义(P<0.05)。临床应用测试结果显示,3D与2D图像系统对冰冻切片异性细胞的诊断准确性方面差异无统计学意义(P>0.05)。结论基于当前视频技术研制的数字显微镜3D图像系统在成像质量上能够满足临床应用的要求。3D图像系统的入景效果对厚层细胞诊断有一定价值,但临床应用测试实验中3D与2D图像系统对冰冻切片内的肿瘤细胞诊断准确性基本相同。今后3D图像系统应在加强稳定性方面继续完善,比如加入自动对焦功能以及配备共轭调解轴等,确保左右两路光学信号的一致性。Objective To investigate the availability and feasibility of three-dimensional video technology for observation and diagnosis of tumor cells. Methods Visual quality evaluation: 10 pathologic doctors viewed 30 pathological section by digital microscope 3D and 2D systems, also used ordinary optics microscope, then compared the imaging quality differences between 3D and 2D systems. Clinical application evaluation: 40 cases of frozen sections from surgery of Department of Thyroid Breast Surgery were selected, chose 1 pathologic doctor for diagnosis of the sections under the system of 3D and 2D, the differences of diagnosis accuracy of tumor cells in frozen section were compared between microscope 3D and 2D systems. Results Visual quality evaluation showed that there was no statistically significant differences between 3D and 2D video systems in the aspects of specimen edge sharpness, color revivification degree, field of view, uniform imaging within the field of vision, vision brightness, noisy point within the field of vision, sense of time delay when mobile vision, non-variation specimen shape(P〈0.05), only two aspects of depth perception/stereo feeling and visual accommodation had statistically significant(P〈0.05). Clinical application evaluation showed that the differences of diagnosis accuracy of tumor cells in frozen section between 3D and 2D systems were not statistically significant(P〈0.05). Conclusion The biomicroscopic three-dimensional video system can meet the basic requirements of clinical cytological display, which developed based on the current video technology. The stereoscopic effect provide by3 D video system is helpful in cell clusters observation, but the clinical application showed the diagnostic accuracy of3 D video system has no difference with the 2D video system. Future improvement of 3D video system should focus on machine stability, such as to improve the two light paths' accordance by addition of the autofocus function and equipping conjugate mediation shaft.
