459例育龄期妇女异常子宫出血病理分析

摘　　要：目的 探讨育龄期妇女异常子宫出血的原因及子宫内膜病理检查的临床意义。方法 回顾性分析2012年1月-2015年12月本所妇科门诊的459例育龄期妇女异常子宫出血病理结果,根据年龄分为A组（18-39岁,174例）和B组（40-48岁,285例）,比较两组各病理类型所占比例。结果 A组发生异常子宫出血的原因以妊娠相关疾病为主,B组以功能性出血（包括子宫内膜增殖症＋增生期子宫内膜）为主。B组的子宫内膜简单型增生症和增生期内膜发生率显著高于A组,差异有统计学意义（P〈0.05）。A组的与妊娠有关内膜发生率显著高于B组,差异有统计学意义（P〈0.05）。结论 不当妊娠、功能失调性子宫出血是造成育龄期异常子宫出血的原因,诊断性刮宫和内膜病理检查对本病的确诊及指导治疗具有帮助。Objective To explore the cause of abnormal uterus bleeding （AUB） of the childbearing age women and clin- ical significance of endometrium pathologic examination. Methods Pathological results of 459 cases of the childbearing age women visited outpatient with abnormal uterine bleeding from January 2012 to December 2015 were retrospectively analyzed,all patients were divided into group A （18 to 39 years old,174 cases） and group B （40 to 48 years old,285 cas- es） according to age.The proportion of different pathological types in the two groups was compared. Results The causes of AUB in group A was mainly related to diseases associated with pregnancy,the causes of AUB in group B was mainly related to dysfunctional uterine bleeding,including endometrial hyperplasia and hyperplastic endometrium.The incidence rate of simple endometrial hyperplasia and hyperplastic endometrium in group B was higher than that in group A,with significant difference （P〈0.05）.The incidence rate of endometrium associated with pregnancy in group A was higher than that in group B,with significant difference （P〈0.05）. Conclusion Improper pregnancy,dysfunctional uterine bleeding are the cause of abnormal uterine bleeding in childbearing age.Diagnostic curettage and endometrial pathological exam- ination is helpful to diagnosis and guiding treatment.
