摘 要:目的 观察负压环境对创面血管生成和血管成熟的影响,并探讨血管生成素/酪氨酸激酶受体-2信号通路在负压环境促进创面血管生成和血管成熟中的作用机制,并且分析成熟微血管增加创面血流灌注的潜在机制.方法 58例软组织缺损患者被随机分为实验组和对照组,分别给予负压和凡士林纱布治疗,并在1、3、7、15 d检测血流量,同时取新鲜肉芽组织使用Western blot分别检测血管生成素-1(Ang-1)、酪氨酸激酶受体-2的磷酸化(pTie-2)、血管生成素-2(Ang-2)、α-平滑肌肌动蛋白(α-SMA)和胶原蛋白Ⅳ(Col Ⅳ)的蛋白表达.结果 负压环境在创面愈合的早期阶段(1~3 d)和晚期阶段(7~15 d)能够改变相关因子的表达,促使实验组第3天Ang-2相对值为1.87±0.35显著高于对照组的1.32±0.23(P <0.05)、pTie-2相对值为0.36±0.02显著低于对照组的0.87±0.04(P <0.05)、Ang-1的相对值为0.34±0.02显著低于对照组的0.78 ±0.07(P<0.05)、Ang-1/Ang-2的比值为0.34±0.02显著低于对照组的0.78±0.07(P <0.05).能够促使实验组第15天Ang-1的相对值为1.89 ±0.24显著高于对照组的1.35±0.22(P <0.05)、α-SMA的相对值为2.56±0.42显著高于对照组的1.68±0.31 (P <0.05)、ColⅣ的相对值为2.32±0.35显著高于对照组的1.36±0.24(P <0.05)、pTie-2的相对值为2.12±0.44显著高于对照组的1.23±0.31(P<0.05)、Ang-1/Ang-2的比值为4.86±0.67显著高于对照组的1.87±0.54(P <0.05)、Ang-2的相对值为0.36±0.05显著低于对照组的0.71±0.06(P <0.05).结论 负压环境在创面愈合的早期阶段能够优先促进血管失稳定微环境的产生,并由此促进创面血管生成数量的增加;在后期阶段,负压环境能够优先促使血管稳定微环境的产生,并且通过血管生成素/酪氨酸激酶受体-2信号通路加速创面血管成熟;同时成熟血管由于周细胞的增加可显著增加创�Objective To study the effect of negative pressure environment on angiogenesis and microvessel maturation and relevant signaling pathway,and explore the potential mechanisms of angiogenin/tyrosine kinase receptor-2 signal pathway promoting angiogenesis and microvessel maturation under negative pressure environment.Methods Successive 58 patients enrolled in this study were treated with negative pressure or petroleum gauze,and at 1st,3rd,7th,and 15th day blood flow perfusion was detected,as well as the Western boltting was applied to detect angiogenin-1 (Ang-1),the phosphorylation of tyrosine kinase receptor-2 (pTie-2),angiogenin-2 (Ang-2),α-smooth muscle actin (α-SMA) and collagen type Ⅳ respectively.Results Negative pressure environment could change the expression of related factorsin the early stages of wound healing (1-3 d) and late stage (7-15 d).In experimental group on day 3,the relative value of Ang-2 was 1.87 ± 0.35 and significantly higher than control group (1.32 ± 0.23,P 〈 0.05),relative value of pTie-2 was 0.36 ± 0.02 and significantly lower than control group (0.87 ± 0.04,P 〈 0.05),relative value of Ang-1 was 0.34 ± 0.02 and significantly lower than control group (0.78 ± 0.07,P 〈 0.05),the ratio of Ang-1/Ang-2 was 0.34 ± 0.02 and significantly lower than the control group (0.78 ± 0.07,P 〈 0.05).In the experimental group on day 15,negative pressure environment leadin to the relative value of Ang-1 was 1.89 ± 0.24 and significantly higher than conrol group (1.35 ± 0.22,P 〈 0.05),relative value of α-smooth muscle actin was 2.56 ± 0.42 and significantly higher than control group (1.68 ± 0.31,P 〈 0.05),relative value of collagen type Ⅳ was 2.32 ± 0.35 and significantly higher than control group (1.36 ± 0.24,P 〈 0.05),the relative value of pTie-2 was 2.12 ± 0.44 and significantly higher than control group (1.23 ± 0.31,P 〈 0.05),the ratio of Ang-1/Ang-2 was 4.86 ± 0.67 and significantly higher than control group �
