摘  要:目的通过频繁单采血小板献血者的血小板数(Plt)、血小板生成素(TPO)及血小板衍生生长因子(PDGF)等因子的变化,探讨频采对献血者巨核细胞的影响。方法随机选取30名连续单采血小板时间>1年(2014年1月-2015年5月),每次间隔15-30 d的献血者作为观察组,30名首次单采血小板的献血者作为对照组。献血前后检测其血常规及TPO、PDGF浓度。结果 1)观察组单采血小板前Plt较对照组明显增多,平均血小板体积(MPV)减小,血小板体积分布宽度(PDW)增大,TPO和PDGF浓度低于对照组:Plt(272.97±42.84)×109/L vs(248.70±31.94)×109/L(P<0.05),MPV(9.91±0.46)f L vs(10.31±0.72)f L(P<0.05),PDW(11.53±0.88)f L vs(10.95±0.77)f L(P<0.01),TPO(66.39±23.26)pg/m L vs(80.42±23.95)pg/m L(P<0.05),PDGF(89.78±16.22)pg/m L vs(99.80±21.41)pg/m L(P<0.05);2)观察组与对照组单采血小板后TPO均较采集前下降:观察组(66.39±23.26)pg/m L vs(57.80±21.11)pg/m L(P<0.01),对照组(80.42±23.95)pg/m L vs(87.92±18.18)pg/m L(P<0.01);3)观察组与对照组采集血小板后PDGF均较采集前下降:观察组(89.78±16.22)pg/m L vs(80.94±14.15)pg/m L(P<0.01),对照组(99.80±21.41)pg/m L vs(87.92±18.18)pg/m L(P<0.01)。结论频采对献血者巨核细胞的刺激增强,使其血小板数更高,消耗更多的TPO及PDGF,因此对频采献血者应关注相关因子的变化,以避免对献血者的健康造成损害。Objective To measure the changes of platelet count( Plt),thrombopoietin( TPO),platelet derived growth factor( PDGF) and other factors in regular platelet donation donors,in order to understand the effect of frequent platelet donation on megakaryocytes of blood donors. Methods Thirty blood donors who donated platelets for more than one year from January 2014 to May 2015 were randomly selected as the observation group. Each interval lasted between 15- 30 days. Another thirty first time blood donors were established as control group. The complete blood count,TPO and PDGF concentration were evaluated before and after blood donation. Results Compared with the control group,the platelet in the observation group significantly increased,MPV reduced and PDW increased than the control group. TPO and PDGF concentration in observation group were lower than the control group: 1). In both the observation group and the control group,TPO decreased compared with those before collection: observation group( 66. 39 ±23. 26) vs( 57. 80 ± 21. 11)( P 0. 01),control group( 80. 42 ± 23. 95) vs( 87. 92 ± 18. 18)( P 0. 01); 3). In both the observation group and the control group,PDGF decreased compared with those before collection: observation group( 89. 78 ± 16. 22) vs( 80. 94 ± 14. 15)( P 0. 01),control group( 99. 80 ± 21. 41) vs( 87. 92 ± 18. 18)( P 0. 01). Conclusion Regular platelet donation donors' megakaryocytes are frequently stimulated,thus inducing a higher blood platelet count and consuming more TPO and PDGF. Therefore,frequent blood donors should pay attention to their physiological changes,in order to avoid damage
