经鼻内镜筛窦径路治疗眶骨爆折的疗效观察

摘　　要：目的 :探讨筛窦径路经鼻内镜手术治疗眶骨爆折的临床疗效。方法 :总结 1998年 10月～ 2 0 0 1年 10月收治眶壁骨折患者 19例 ,均在鼻内镜下 ,行眶骨骨折整复术。结果 :经 6～ 12个月随访眶骨骨折 19例 ,术后复视消失 ,眼球凹陷改善。结论 :经鼻内镜筛窦径路手术治疗眶骨爆折 ,入路简捷 ,操作方便 ,眶内侵袭少 ,无面部疤痕 ,疗效可靠。Objective:To evaluate the treatment effect of the orbital bust fracture by surgery of nasal endoscopy.Methods:Nineteen patients with orbital bust fracture were treated by surgery of nasal endoscopy in our department from 1998 to 2001.There were 12 males and 7 females,ranging in age from 16 to 55 years old.The CT feature of these patients was endophthalmos and 'teardrop shape' in sinus cavity .Under the nasal endoscope,the ethmoid sinus was opened by Messerklinger′s approach,then the impacted orbital lipid and medial rectus in the ethmoid sinus were pushed into the orbit,and the pores were filled with silicone.Result:After operation,the diplopia and endophthalmos were corrected with a follow\|up of 6 to 12 months.Conclusion:Our result indicated that the restitution of the orbital bust fracture treated by approach of the ethmoid sinus under nasal endoscopy had advantages such as no scars,light operative injury,less complications,and great safety.
