摘 要:目的探讨N-乙酰半胱氨酸(NAC)联合氨溴索对老年慢性阻塞性肺疾病(COPD)急性加重期患者氧化应激的影响。方法 COPD急性加重期患者70例随机分成A、B、C 3组,A组23例给予综合治疗加NAC 600 mg,口服,2次/d;B组24例给予综合治疗加氨溴索片60 mg,口服,3次/d;C组23例给予综合治疗加NAC联合氨溴索片,剂量同上,疗程为14 d。比较3组患者治疗前后血清超氧化物歧化酶(SOD)、丙二醛(MDA)含量及肺功能的变化。结果 3组患者治疗后外周血SOD均增加,MDA均降低,1 s用力呼气容积占预计值的百分比(FEV1%)及l s用力呼气容积占用力肺活量百分比(FEV1/FVC%)均增加,与治疗前比较差异有统计学意义(P<0.05),C组SOD、MDA、FEV1%、FEV1/FVC%改善较A、B组更明显(P<0.05)。结论NAC联合氨溴索能够有效改善老年COPD急性加重期患者氧化应激和肺功能,可能与提高机体抗氧化能力有关。Objective To investigate the effect of N-acetylcysteine combined with ambroxol on oxidative stress in elderly patients with chronic obstructive pulmonary disease( COPD) acute exacerbation. Methods Seventy patients with COPD acute exacerbation were randomly divided into three groups( group A,n = 23; group B,n = 24 and group C,n =23). Group A was treated with general treatment and N-acetylcysteine 600 mg,twice a day; Group B was treated with general treatment and ambroxol 60 mg,three times a day; And group C was treated with general treatment and the combination of N-acetylcysteine and ambroxol with the same dosage. The total observation period was 14 days. Plasma levels of SOD,MDA and lung function of three groups were compared before and after treatment. Results After 14 days of treatment,the plasma levels of SOD,MDA and the lung function were improved significantly in the three groups,especially in group C( P〈0. 05). Conclusions The treatment of N-acetylcysteine combined with ambroxol can relieve the oxidative stess and lung function in the patients with COPD acute exacerbation,probably by improving the capacity of body's antioxidant capacity.
