摘  要:目的探索前列地尔对老年早期糖尿病肾病(DN)患者炎性因子、肾功能及血管内皮功能的影响。方法 144例老年早期DN患者进行研究,并按照就诊顺序随机分成2组。对照组72例予常规内科治疗,观察组72例同时加用前列地尔干乳剂治疗,治疗前后检测2组血清炎性因子、肾功能及血管内皮功能各项指标并比较其变化。结果治疗后,观察组患者血清肌酐(SCr)、尿素氮(BUN)、尿蛋白排泄率(UAER)、β2微球蛋白(β2-MG)均较治疗前明显下降(P<0.05),且各项指标显著低于对照组(P<0.05)。治疗后,观察组患者白介素(IL)-6、肿瘤坏死因子α(TNF-α)、高敏C反应蛋白(hs-CRP)均较治疗前明显下降(P<0.05),且各项指标显著低于对照组(P<0.05)。治疗后,观察组患者内皮素-1(ET-1)、血管性血友病因子(v WF)均较治疗前明显下降,一氧化氮(NO)较治疗前明显增加,且各项指标与对照组比较,差异有统计学意义(P<0.05)。结论前列地尔用于老年早期DN治疗能够有效降低患者炎性因子水平,改善其肾功能及血管内皮功能,具有较高的应用价值,值得借鉴和推广。Objective To explore the effects of alprostadil on inflammatory factor,renal function and vascular endothelial function in treatment of elderly patients with early diabetic nephropathy( DN). Methods 144 cases of elderly patients with early DN were randomly divided into two groups. The control group( 72 cases) were treated with conventional medical treatment,and the observation group( 72 cases) received alprostadil dry emulsion additionally. The serum levels of inflammatory factor,renal function and vascular endothelial function indexes of two groups before and after treatment were compared. Results After treatment,serum creatinine( SCr),urinary albumin excretion rate( UAER),blood urea nitrogen( BUN) and β2-microglobulin( β2-MG) in the observation group were significantly decreased( P〈0. 05),and the indexes were significantly lower than those in the control group( P〈0. 05). After treatment,the levels of interleukin( IL-6),tumor necrosis factor-α( TNF-α) and high sensitivity C reactive protein( hs-CRP) were significantly decreased in the observation group( P〈0. 05),and the indexes were significantly lower than those in the control group( P〈0. 05).After treatment,the levels of von willebrand factor( v WF) and endothelin-1( ET-1) were significantly decreased,and the level of NO was significantly increased in the observation group. The levels of above indicators in the observation group were significantly different from those in the control group( P〈0. 05). Conclusions Alprostadil can effectively reduce the level of inflammatory factors,improve the renal function and vascular endothelial function for early stage of DN in the elderly.
