医防合作对加强结核病控制的影响

摘　　要：目的探讨医防合作对提高肺结核患者发现工作的影响。方法分析2005-2013年济源市综合医疗机构网络直报、转诊和结防机构追踪情况,评价医防合作对肺结核发现工作的贡献。结果济源市综合医疗机构实际报告率从2005年的32.73/10万逐渐提高,最高达123.98/10万;转诊到位率从2005年的47%逐年提高,最高达90.73%,追踪到位率从2005年的20.87%逐渐提高,最高达92.95%以上,总体到位率从开始的58.06%逐渐提高,最高达97.53%以上(趋势χ2检验,P<0.01);活动性患者发现的贡献率从33.06%提高最高70%以上(趋势χ2检验,P<0.05),涂阳肺结核发现患者的贡献率从27.21%提高到最高时78%以上(趋势χ2检验,P<0.01);结论 2005-2013年河南省济源市综合医疗机构在肺结核发现工作中起到重要作用,医防合作的开展明显提高了肺结核患者的发现水平。Objective To explore the impact of cooperation between CDC and hospitals on identification of pulmonary tuberculosis（ TB） patients. Methods The status of direct reports of epidemics network,referral and TB track were analyzed from 2005 to 2013 in Jiyuan medical organization,Henan province,to evaluate the contribution of the cooperation between CDC and hospitals on identification of pulmonary. Results From 2005 to2013,the actual report rate of TB was increased from 32. 73 to 106. 68 per 100 thousand,and the top rate was123. 98 per 100 thousand in comprehensive medical organizations. The actual rate of referral from 47% in 2005 was increased gradually,and the top rate was over 90. 73%; the rate of track from 20. 87% in 2005 was increased and the top rate was over 92. 95%; the rate of general admission from 58. 06% of beginning was increased and the top rate was over 97. 53%（ trend χ2test. P 0. 01）. The rate of contribution from collaboration between CDC and hospital,it was from 33. 06% to over 70% at top on identification of active TB（ trend χ2test. P 0. 05） and it was from 27. 21% to over 78% at top on test positive of sputum smear（ trend χ2test. P 0. 01）. Conclusion In 2005 to 2013,comprehensive medical organizations played an important role on identification of pulmonary TB,and the cooperation between CDC and hospital obviously improved the rate of identification works.
