摘  要:目的探讨终末期糖尿病肾病患者血液透析患者阿司匹林抵抗现象。方法选择终末期糖尿病肾病维持性血液透析治疗患者共67例(糖尿病肾病血液透析患者组),另外分别选择同期入院的糖尿病肾病非血液透析患者共70例(糖尿病肾病非血液透析组)和肾功能正常的糖尿病患者70例(糖尿病对照组),检测二磷酸腺苷诱导血小板最大聚集率(ADP-PAG)和花生四烯酸诱导血小板最大聚集率(AA-PAG),分析阿司匹林抵抗(AR)情况。结果糖尿病肾病血液透析组患者AR发生的比率显著高于糖尿病非血液透析组和糖尿病对照组患者;糖尿病肾病血液透析组患者ADP-PAG水平和AA-PAG水平均显著高于糖尿病肾病非血液透析组和糖尿病对照组。结论糖尿病肾病血液透析患者是阿司匹林抵抗发生的高危人群,对该人群给予阿司匹林时应注意药物抵抗的问题。Objective To investigate the aspirin resistance in end- stage diabetic nephropathy hemodialysis patients. Methods A total of 67 cases of end- stage diabetic nephropathy hemodialysis patients( diabetic nephropathy hemodialysis patients group) were collected. In addition to this,a total of 70 cases of non- hemodialysis patients with diabetic nephropathy during the same period of hospitalization( diabetic nephropathy in non- hemodialysis group) and 70 diabetes patients( diabetic control group) were selected. Platelet aggregation was undertaken and the situation of aspirin resistance was analyzed. Results AR occurrence in the proportion of patients with diabetic nephropathy hemodialysis was significantly higher than the non- hemodialysis diabetic nephropathy and diabetic patients. ADP- PAG levels and AA- PAG levels in the patients with diabetic nephropathy hemodialysis group were significantly higher than non- hemodialysis diabetic nephropathy group and diabetic control group.Conclusion Diabetic nephropathy hemodialysis patients have a high risk of aspirin resistance occurring. We should pay attention to the problem of aspirin resistance.
