钙离子拮抗剂治疗中度哮喘的临床观察

摘　　要：目的探讨钙离子拮抗剂(维拉帕米)治疗中度哮喘患者的临床疗效。方法将符合纳入要求的40例中度哮喘患者以随机数字表法分为两组,每组各20例,对照组采用予少剂量糖皮质激素、氨茶碱、β2受体激动剂等常规治疗,观察组采用在常规治疗基础上加用维拉帕米治疗,两组分别在治疗的第0、2、4、6、8、10周记录哮喘控制水平及肺功能。结果两组哮喘治疗有效率随着治疗的时间的延长而升高,在治疗后的第8、第10周两组的有效率有统计学差异(P<0.05);两组患者肺功能状况好转,观察组的FEV1%上升速度较对照组更加明显,在治疗第4周后两组的FEV1%的差异均有统计学意义(均P<0.05),同时观察组的PEF变异率下降速度较对照组快,在治疗后第6周后两组的PEF变异率均有差异性(均P<0.05)。结论钙离子拮抗剂(维拉帕米)能够提高支气管哮喘的治疗效果,是潜在治疗支气管哮喘的一种有效新途径。Objective To investigate the clinical effect of calcium antagonist （verapamil） in the treatment of moder- ate asthma. Methods 40 patients with moderate asthma meeting the inclusion criterion were randomly assigned into two groups by random digit table, 20 cases in each group. The control group was treated by standard therapies with corticoste- folds, aminophylline, long - acting beta - agonist, while the observation group additionally with verapamil based on the stand- ard therapies. The asthma control level and lung function of the two groups were recorded at the 0, 2nd, 4th, 6th, 8th and 10th week, respectively. Results The effectiveness rates of the two groups improved over treatment time, significantly at the 8th week and 10th week （P 〈 0. 05）. The two groups both had better lung function after treatment. FEV1% of the observa- tion group was faster in rising velocity compared to the control group, and FEV1% of two groups showed a significant differ- ence after 4 weeks of treatment （ P 〈 O. 05 ）. The observation group was faster at the descending speed of PEF mutational rate, and the PEF ratios of two groups had a significant difference after 6 weeks of treatment （ P 〈 0. 05 ）. Conclusion Cal- cium antagonist （verapamil） can improve the therapeutic effect on bronchial asthma as a new, potential and effective therapy for asthma.
