摘　　要：目的观察氟哌噻吨美利曲辛联合匹维溴铵长疗程治疗腹泻型肠易激综合征(IBS)的疗效和安全性。方法将147例腹泻型IBS患者随机分为治疗组74例及对照组73例,治疗组采用氟哌噻吨美利曲辛片联合匹维溴铵治疗,对照组采用匹维溴铵治疗,观察两组腹痛和腹胀时间变化和排便次数的变化以及不良反应发生情况,并比较两组治疗前后的汉密尔顿焦虑量表(HAMA)及汉密尔顿抑郁量表(HAMD)评分。结果治疗组临床治愈率为70.83%,显著高于对照组的27.14%(P<0.01);治疗后两组患者腹痛及腹胀的持续时间和排便次数均减少,且治疗组的持续时间和次数减少更明显(P<0.05);治疗组及对照组的HAMA评分及HAMD评分均有下降,且治疗组评分下降更明显(P<0.05);两组患者匀未发现有肝、肾等功能损害。结论氟哌噻吨美利曲辛片联合匹维溴铵长疗治疗腹泻型IBS疗效显著,且不良反应少,值得临床推广。Objective To observe the clinical efficacy and safety of flupenthixol melitracen combined with pinaverium bromide in the long course treatment of diarrheic irritable bowel syndrome（ IBS- D）. Methods A total of 147 patients with IBS- D were randomly divided into combined treatment group（ 74 cases） and control group（ 73 cases）, both groups received pinaverium bromide by oral administration, and the combined treatment group received additional flupenthixol melitracen. The time changes of abdominal pain and distention, stool frequency and adverse reactions of the two groups were observed, and the HAMA and HAMD scores before and after treatment were compared. Results The total effective rate of the combined treatment group was 70. 83%, which was obviously higher than27. 14% of the control group（ P〈 0. 01）, The time changes of abdominal pain and distention, stool frequency, HAMA and HAMD scores of combined treatment group had obviously higher than those of the control group（ P〈 0. 05）. The HAMA and HAMD scores of both groups were decreased after treatment, and the combined treatment group decreased more significantly（ P〈 0. 05）. Liver and kidney impairment had not been found in both groups. Conclusion Flupenthixol melitracen combined with pinaverium bromide has clear efficacy in treating diarrheic IBS- D with few adverse reactions, which is worthy of clinical promotion.
