横穿钉和纽扣钢板重建前交叉韧带的近期效果比较

摘　　要：[目的]比较使用横穿钉和纽扣钢板重建前交叉韧带的近期效果.[方法]分析本科室2012年至2014年重建前交叉韧带患者共68例,采用取自体腘绳肌、股薄肌腱单束重建,股骨端使用用横穿钉固定36例,使用纽扣钢板固定32例.对术前术后膝关节的稳定性及功能通过Lachman试验、Lysholm评分和前抽屉试验进行评价.[结果]患者随访12～ 16月,横穿钉组和纽扣钢板组的Lysholm评分术前分别为：（40.31±13.26）分和（43.21±12.57）分,术后分别为（92.26±5.08）分和（91.13±6.15）分,两组术后评分均优于术前,两组术后评分差异无统计学意义.[结论]使用横穿钉和纽扣钢板重建前交叉韧带都能改善患者膝关节的稳定性,两者近期效果无差别.[ Objective ] To observe the recent clinical efficacy of arthroscopic anterior cruciate ligament （ACL） reconstruction with autogenous hamstring tendon using cross-pin with button plate. [ Method ] 68 patients with anterior cruciate ligament injury were treated between 2012 and 2014. They were operated with arthroscopic reconstruction of anterior cruciate ligament with autogenous hamstring tendon. 36 of them were used cross-pin while 32 of them were used button plate in the femoral side fixation. Lachman test, anterior draw test and Lysholm scoring were used to evaluate pre-operative and post-operative knee stability and function. [Result] The follow-up duration ranged from 12 to 16 months. Lysholm score were 40.31 ± 13.26 and 43.21 ± 12.57 in cross-pin group and button plate group before operation, 92.26 ± 5.08 and 91.13 ± 6.15 after operation. The score after operation were higher than before operation in both groups, with no significant differences between the two groups. [ Conclusion ] The stability of the knee joint of patients who received ACL reconstruction by using both cross-pin and button plate can be improved. There were no significant differences in the recent clinical efficacy.
