摘 要:目的:探讨血红蛋白自动分析仪在糖尿病合并高血脂患者糖化血红蛋白(Hb A1C)及血液流变学联合检测中的应用。方法:选取84例2型糖尿病伴高脂血症患者纳入观察组,同时选择84名健康体检者为对照组,采用血红蛋白自动分析仪对两组进行血液流变学及血糖检测,并进行对比,分析联合检测Hb A1C及血液流变学对糖尿病合并高血脂患者的临床意义。结果:观察组与对照组血脂指标检测结果比较,观察组患者血清胆固醇(TC)、甘油三酷(TG)及低密度脂蛋白(LDL)等水平均明显高于对照组,差异显著(t=9.70,t=11.37,t=29.60;P<0.05),而高密度脂蛋白(HDL)无统计学差异;观察组与对照组HbA1C及血液流变学5项指标检测结果比较,观察组HbA1C、全血高切黏度及全血低切黏度等水平均明显高于对照组,差异显著(t=3.16,t=3.32,t=3.08;P<0.05);观察组全血低切黏度、高切黏度与HbA1C间呈高度正相关(r=0.948,r=0.951;P<0.01);血浆黏度与HbA1C间也存在明显相关性(r=0.863,P<0.01)。结论:联合检测血液流变学指标与HbA1c可有效反映糖尿病合并高血脂患者病情变化,对临床诊治意义重大,值得进一步推广使用。Objective:To investigate the application of combined detection glycated hemoglobin and blood rheology with the diabetic patients complicated with hyperlipidemia.Methods: 84 cases were selected as the observation group with type 2 diabetes mellitus patients with hyperlipidemia, while 84 cases healthy patients were selected as a control group. All patients were tested in rheology, blood sugar, and comparing analysis of combined detection of glycated hemoglobin and blood rheology of diabetic patients with clinical significance in patients with high cholesterol.Results: Test results of patients with serum lipids showed that of the patients in the observation group were significantly higher than the patients in the control group with TC, TG, LDL and so no. The difference was statistically significant(t=9.70,t=11.37,t=29.60,P〈0.05), while HDL was not statistically significant. The test results of these five indicators in observation group and the control group of patients with HbA1C and blood rheology showed that the observation group with HbA1C, high shear whole blood viscosity and whole blood levels were significantly higher than the patients with low shear viscosity. The difference was statistically significant(t=3.16,t=3.32,t=3.08,P〈0.05). The observation group with low shear whole blood viscosity and high shear viscosity highly positive correlated with HbA1C association(r=0.948,r=0.951,P〈0.01). There is significant correlation between plasma viscosity and HbA1C(r=0.863,P〈0.01).Conclusion: It is a considerable clinical significance to the combined detection of blood rheology and HbA1c effective expression in patients with diabetes mellitus high cholesterol condition changes and it is worth popularizing in clinical application.
