摘  要:目的研究红细胞分布宽度(RDW)与冠状动脉扩张(CAE)相关性。方法以我院2014年10月至2016年1月因胸痛就诊的280例患者为研究对象,根据冠状动脉造影结果分为冠状动脉扩张组(A组)、冠心病组(B组)、冠状动脉扩张伴冠心病组(C组)及冠状动脉正常组(D组),收集整理各组临床资料,入院24h内测定各组RDW、血脂水平、相关炎性指标[如高敏C反应蛋白(hs-CRP)、白细胞计数(WBC)、白细胞介素(IL)-6、肿瘤坏死因子(TNF)-α、同型半胱氨酸(Hcy)]等,分析RDW与CAE的相关性。并通过Mariks分型评价CAE严重程度,分析其与CAE的相关性。结果 A、B、C组与D组在与吸烟史、糖尿病病史、RDW、WBC、hs-CRP、IL-6、TNF-α、Hcy水平上比较差异有统计学意义(P<0.05)。CAE轻中度、重度组RDW分别为(12.0±0.6)%、(12.6±0.9)%,显著高于D组的(11.8±0.6)%(P<0.05)。Spearman相关性分析得出RDW与WBC、hs-CRP、IL-6、TNF-α、Hcy显著相关(P<0.05)。多因素回归分析发现其独立变量为RDW、WBC、Hcy。结论RDW与CAE及其病变程度显著相关,且两者相关可能与炎症机制有关。Objective To study the correlation between red blood cell distribution width(RDW)and coronary artery ectasia(CAE).Methods Two hundred and eighty patients who were treated in our hospital because of chest pain from October 2014 and to January 2016 were treated as the study subjects.According to the results of coronary angiography,the patients were divided into CAE group(group A),coronary heart disease group(group B),CAE complicated with coronary heart disease group(group C)and normal coronary artery group(group D).The clinical data of the groups were collected.In 24 hafter admission,RDW,blood lipid levels and related inflammatory indexes(such as hs-CRP,WBC,IL-6,TNF-α,Hcy),etc.were determined and the correlation between RDW and CAE was analyzed.The severity of CAE was evaluated by Mariks typing and its correlation with CAE was analyzed.Results The differences in smoking history,history of diabetes,RDW,WBC,hs-CRP,IL-6,TNF-αand Hcy levels among group A,B,C and D were statistically significant(P〈0.05).RDW in mild to moderate and severe CAE group[(12.0±0.6)%,(12.6±0.9)%]were significantly higher than that in group D[(11.8±0.6)%](P〈0.05).Spearman correlation analysis showed that RDW was significantly correlated with WBC,hs-CRP,IL-6,TNF-αand Hcy(P〈0.05).Multivariate Logistic analysis showed that the independent risk factors of CAE were RDW,WBC and Hcy.Conclusion RDW is significantly correlated with CAE and the correlation may be related to inflammatory mechanism.
