空心螺钉内固定治疗Lisfranc关节损伤的效果

摘　　要：目的探讨切开复位空心螺钉结合克氏针固定治疗Lisfranc关节损伤的临床效果。方法选取2010-2014年收治的Lisfranc关节损伤患者54例,随机均分为观察组与对照组,每组27例。组间患者年龄、性别比例、Myerson分型类似。观察组患者行切开复位空心螺钉结合克氏针固定,对照组采用切开复位克氏针固定。比较2组患者的治疗效果及并发症发生率。结果观察组患者疼痛评分、手术时间及愈合时间分别为(4.53±1.12)分、(130.28±16.57)min、(24.33±4.15)d,稍高于对照组(4.22±1.31)分、(125.37±13.81)min、(22.78±3.7)d,但差异无统计学意义(P>0.05)。2组患者治疗后AOFAS评分:观察组为(83.77±14.29)分、对照组为(76.23±12.26)分,差异有统计学意义(P<0.05)。观察组患者治疗优良率为85.2%,显著高于对照组(59.2%)(P<0.05)。观察组切口感染、骨性关节炎、骨筋膜综合征发生率分别为(1例,1例,1例,11.11%),与对照组(1例、1例、2例、14.81%)相比差异无统计学意义(P>0.05)。结论切开复位空心螺钉结合克氏针固定治疗Lisfranc关节损伤,效果好,并发症发生率较低。Objective To assess the effects of treatment of Lisfranc injury with open reduction and canulated screw fixed. Methods54 patients with Lisfranc injury were collected from 2010 - 2014 in our hospital,all patients were divided into observation group and control group randomly. Patients in observation group were treated with open reduction,canulated screw and kirschner wire fixed; while open reduction and kirschner wire fixed were performed in control group. The two groups were compared in terms of treating indicators,therapeutic effect and complications. Results The pain score,mean operation time and mean healing time were respectively [（ 4. 53 ± 1. 12） points、（ 130. 28 ±16. 57） minutes、（ 24. 33 ± 4. 15） days] vs [（ 4. 22 ± 1. 31） points、（ 125. 37 ± 13. 81） minutes、（ 22. 78 ± 3. 7） days],and no significant differences were found in treating indicators between two groups（ P〈0. 05）. The FOFAS score of observation group [（ 83. 77 ± 14. 29） points] was significantly higher than control group [（ 76. 23 ± 12. 26） points]（ P〈0. 05）,and significant differences were found in the excellent and good rate between observation group and control group（ 85. 2% vs 59. 2%）（ P〈0. 05）. The complications rate of observation group and control group was respectively 11. 11% and 14. 81%,no significant difference were found between groups（ P〈0. 05）. Conclusion Treatment of Lisfranc injury with open reduction,canulated screw and kirschner wire fixed could improve the clinical therapy and reduce the complication rate.
