2012年～2014年永久起搏器植入术围手术期抗菌药物使用调查分析

摘　　要：目的为规范某院外科围手术期抗菌药物的使用,对该院药学部2013年行永久起搏器植入术患者围手术期抗菌药物的使用进行用药干预,现统计分析2012年~2014年永久起搏器植入术患者围手术期抗菌药物使用情况,以评估2013年后临床药师干预的效果。方法选取2012年~2014年行永久起搏器植入术的701例患者进行干预对照研究。非干预组为2012年行永久起搏器植入术患者;干预组为2013年、2014年行永久起搏器植入术患者。结果干预后,患者预防性使用抗菌药物的给药时机和疗程愈趋合理;人均药费由2012年2 273.17元下降到2014年2215.76元;人均抗菌药物费用由2012年1 228.15元下降到2014年932.92元。结论药学部干预永久起搏器植入术围手术期抗菌药物使用办法可行、有效,能显著提高永久起搏器植入术围手术期预防用抗菌药物合理使用率,降低药品费用,保证抗菌药物应用安全、有效、经济。Objective To standardize the using principles of antibiotics in perioperative period in hospital, the Pharmacy intervened the using of antimicrobial for permanent pacemaker implantation in 2013. In order to assess the effect of the 2013 pharmaceutical interventions, Statistics of antimicrobial drug use in permanent pacemaker implantation in perioperative period were made from 2012 to 2014. Methods The data of medications of two patients group in hospital from 2012 to 2014 were collected and compared, totally 701 cases. Cases in March of intervention group were inquired as the baseline survey,non-intervention group for the 2012 line of permanent pacemaker implantation in patients; the intervention group was in 2013, in 2014 the line of permanent pacemaker implantation in patients. Results After intervention, the reasonable rate of antimicrobial giving time for permanent pacemaker implantation were increased, and so does the course of treatment; average drug fees were fell from2 273.17 yuan to 2215.76 yuan; the average cost of antibiotics by the 1228.15 yuan fell to 932.92 yuan. Conclusion The integrated intervention managements are effective and feasible. This method can significantly reduce the prophylactic use rate of antibiotics in permanent pacemaker implantation in perioperative prophylactic, improve the rational rate of antimicrobial drugs, decreed drug cost,which is safe, effective and economical for the promotion of clinically use of antimicrobial drugs.
