摘 要:目的探讨促红细胞生成素(erythropoietin,EPO)对横纹肌溶解大鼠急性肾损伤(acutekidneyinjury,AKI)的细胞凋亡的影响及其机制。方法SD大鼠72只,随机分4组,正常对照组、AKI组、AKI+EPO组、AKI+盐水组,分别于第24h、48h、72h采血检测血清中肌酐(Cr)、尿素氮(BUN)水平;留取肾组织行HE染色,光镜下观察肾脏损伤程度及Paller法进行肾小管损伤评分;TUNEL法检测肾小管上皮细胞凋亡情况;RT—PCR及qRT—PCR法检测P—Akt、Bcl-2、BaxmRNA的表达情况。结果与正常对照组比较,AKI组及AKI+盐水组血cr、BUN水平显著上升(P〈0.05),Paller法评分增高,肾小管上皮细胞凋亡比例增加,抗凋亡因子P—AktmRNA、Bcl-2mRNA及促凋亡因子BaxmRNA表达增强(P〈0.05);AKI+EPO组与AKI组和AKI+盐水组相比血Cr、BUN水平显著降低(P〈0.05),Paller法评分下降,肾小管上皮细胞凋亡比例减少,抗凋亡因子P—AktmRNA、Bcl-2mRNA表达增强而促凋亡因子BaxmRNA表达减弱(P〈0.05)。结论EPO可以减轻横纹肌溶解AKI,其机制与抑制细胞凋亡有关。Objective To observe if Erythropoietin(EPO) can cause the inhibition of cell apoptosis in rhabdomyolysis-induced Acute Kidney Injury(AKI) in rats and investigate the mechanism. Methods 72SD rats were randomlydivided into four groups: Normal control group , AKI group , AKI +EPO group and AKI-saline group, The renal function and kidney histopathology were investigated, controls. Serum creatinine and blood urea nitrogen were tested at 24-hour,48-hour,72-hour after the the establishment of model. HE staining and paller score to investigate pathologic change. The renal tubular epithelial cell apoptosis was revealed by TUNEL staining at each time point. The expression of P-Akt, Bcl-2 and Bax mRNA was detected by RT-PCR and qRT-PCR. Results Compared with Normal control group, AKI group and AKI-saline group, have higher serum creatinine, blood urea nitrogen and Paller score(P〈0.05). Kidney tissue of AKI group and AKI-saline group had obvious tubule cell apoptosis, EPO+AKI groups had less apoptosis. P-Akt,Bcl-2 mRNA and Bax mRNA was increased in AKI group and AKI-saline group compared normal control group, Compared with AKI group and AKI-saline group,AKI + EPO groups have lower Serum creatinine, blood urea nitrogen and Paller score(P〈0.05). Kidney tissue of AKI+EPO group had less apoptosis.P-Akt, Bcl-2mRNA expressed more and Bax mRNA expressed less in AKI+EPOgroups than AKI group and AKI-saline group. Conclusions EPO can reduce tubule cell apoptosis in rhabdomyolysis-induced acute kidney injury in Rat.
